Description

  • For a limited time MaineHealth is offering up to $10,000 sign on for Registered Nurses with 0-1.99yearsof experience and $20,000 for experienced Registered Nurses with 2+years of experience!
  • Eligible candidates are hired (offer accepted) into a Full or Part time RN position.
    Bonus amount prorated for Part time hires, per diem hires are ineligible.
  • Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 6 months separation from employment.

This is a full-time, 32 hour per week position.

Your Role:
CHANS Home Health & Hospice | MaineHealth is searching for an RN Case Manager. The Hospice RN Case Manager provides high reliable, evidence-based, patient-family centered quality care for terminally ill patients in their homes. The Hospice RN Case Manager develops the plan of care in collaboration with patient and family goals. Our team is dedicated to our community, and we are passionate about what we do.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: Graduate of an accredited School of Nursing required; BSN preferred.
2. License/Certifications: Current applicable state(s) license as a Registered Professional Nurse required. Current Driver's license with vehicle available for work covered by liability insurance as specified by agency policy. Current BLS Certification or must complete within 30 days of start date.
3. Experience: Previous experience required. Demonstrates competency in all essential functions of a Registered Nurse - Case Manager I role.
