Job Description

Pay-$17.00-28.00/hr

Location California: Commutable distance to San Jose, Walnut Creek, Santa Rosa

POSITION SUMMARY: The Incident Coordinator/Investigator reviews all incidents to ensure that incidents requiring investigation will be thoroughly investigated and that all other incidents receive appropriate follow-up and corrective action, conducts assigned investigations and internal evaluations of all Elwyn California's Adult Intellectual and/or Developmental Disabilities (IDD) services to ensure quality care, performs assessments of all services and makes recommendations for improvement in all aspects of service delivery, and participates in on call responsibilities.

DUTIES AND RESPONSIBILITIES:

In accordance with the procedures and expectations outlined in Elwyn's Incident Management (IM) Policy, the Incident Coordinator/Investigator:


  • Reviews all Special Incident Reports (SIRs) to determine appropriate and required action(s)
  • Ensures appropriate and timely filing and entry into designated systems within twenty-four (24) hours and to designated reporting regional centers and agency(ies) per established deadlines, contracts, and regulations, where applicable
  • Reviews the SIR with the appropriate service area Quality Improvement (QI) Specialist (QIS) or Associate Director of QI, where appropriate, for possible support needed in guiding the Regional Director and/or Home Administrator with appropriate corrective action/plan
  • Oversees incident corrective action and follows up with the Regional Director, or designee, if it is missing
  • Where applicable, follows up with the home administrator to guide submission of related necessary reports with appropriate regional center, outside agency(ies), including but not limited to the California Department of Social Services' (CDSS) Adult Protective Services, Community Care Licensing, and/or law enforcement to ensure regulatory compliance
  • Develops a tracking sheet for incident tracking, and tracks SIRs for trending (e.g., abuse/neglect cases, falls, etc.) to determine training or other necessary follow-up
  • Analyzes the SIRs monthly and reports incidents and trends to program leadership quarterly in Continuous Quality Improvement meetings to improve processes and procedures to maintain regulatory compliance
  • Trains all personnel on incident management and mandated reporting to ensure processes and best practices are followed
  • Leads and/or participates in all policy development (e.g., operations, risk management, quality improvement) for Elwyn CA
  • Conducts Investigations by:


    • Identifying investigatory question(s)
    • Adhering to the Health Insurance Portability and Accountability Act (HIPAA) and other State of California privacy laws, regulations, and requirements during the completion of interviews and at all times during the collection of documentation and information,
    • Securing all statements, evidence, etc., appropriately, and securing the scene, when possible
    • Interviewing victim(s), witness(es), target(s) as soon as possible, once the investigation begins
    • Collecting direct, demonstrative, and documentary evidence, including collecting and managing physical evidence and ensuring Chain of Custody
    • Identifying, sorting, classifying, reconciling, and analyzing evidence collected
    • Completing an investigation report
    • Presenting the investigatory results and report to the administrative team


  • Adheres to the elements of objectivity, speed, and thoroughness while conducting investigations to come to the best resolution possible
  • As Incident Coordinator, ensures administrative team uses a preponderance of the evidence standard to determine substantiated findings
  • Conducts internal evaluations (e.g., spot checks) as part of the IDD Services' Continuous Quality Improvement (CQI) program to ensure best practices
  • Reports observed Quality Improvement needs to the Executive Director of QI in lieu of the Director of QI to create more effective processes/best practices
  • Consistently demonstrates the ability to anticipate future needs or problem areas and suggest effective action
  • Conducts interviews with individuals supported, their families, staff members, advocates, etc. to verify quality services are being delivered
  • Evaluates all aspects of programming and ensures all policies and procedures are followed and enforced
  • Obtains and maintains current knowledge of regulatory standards and updates relative to Elwyn CA programs
  • Maintains a current Investigators manual (received as part of formal training)
  • Completes Peer Reviews of investigations and participates in the review of own investigations (e.g., lessons learned) in order to become more proficient in job duties
  • As a mandated reporter, demonstrates responsibility through responsiveness to others and competent follow up on quality improvement matters requiring additional attention
  • Travels to sites where needed to conduct investigations
  • Obtains and maintains current Investigator training and credentialing
  • On Call Incident Management
  • Performs other duties as assigned



EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:



  • High school diploma or graduate equivalency diploma (GED); bachelor's degree in the field of human services or related field preferred
  • Two (2) years of experience as an investigator or equivalent investigatory experience
  • Obtain and maintain Pennsylvania Certified Investigator status (if not certified investigator upon hire, must complete the certified investigator training within one (1) year of hire) regardless of state or program allowing for a consistent and thorough understanding of the investigatory process
  • One (1) year experience working directly with people with intellectual, developmental, or medical disability(ies)
  • Knowledge of investigation policies, procedures, and standards required per applicable regulations, payors, and state agencies
  • Must possess high ethical standards, a reputation of honesty and credibility, versatile interviewing abilities, good judgment and flexibility
  • Highly developed organizational skills
  • Excellent customer interaction, collaboration, presentation, and written and verbal communication skills
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; ability to learn and independently use internal and external investigation reporting and management systems
  • Experience with an Electronic Health Record preferred
  • Demonstrated strong attention to detail
  • Knowledge of the population Elwyn serves including adults with behavioral health issues, and intellectual, developmental, and physical disabilities
  • Ability to independently write comprehensive reports in English with proper grammar and spelling; ability to present findings clearly and objectively
  • Ability to work in a potentially emotion-filled environment
  • Ability to conduct interviews in a variety of locations, including but not limited to licensed facilities, group homes, and community settings; availability to travel for business, to work flexible work hours, as needed with some overnight travel
  • Ability to work in environments where individuals with behavioral issues may be present
  • Completion of Direct Support Professional (DSP) I training during first six (6) months of employment and DSP II during second year of employment, or earlier
  • Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
