Primary Care Clinical Manager - Jefferson Center City

The Program Manager, Primary Care Clinical Initiatives, plays a pivotal role in the design, implementation, scaling, and oversight of primary care quality and clinical standardization initiatives across Jefferson Medical Group. This leadership position is responsible for developing and implementing priority programs to enhance the quality of care, patient experience, clinical outcomes, and operational best practices. This work will require close collaboration with enterprise and regional leaders to engage and support providers and staff across our large primary care system

  • Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson
  • Implements and manages strategic initiatives and programs to improve clinical services across Jefferson Primary Care
  • Partner closely with subject matter experts, clinical and operational leaders, nursing leadership, population health, informatics leadership, and other key collaborators to promote systems redesign aligned with priority initiatives
  • Ensure data-based approach to program goals and engage with clinical and operational leaders to support success in achieving program targets, ensuring accountability to meeting defined goals on schedule
  • Create and disseminate new workflows and approaches to care at regional and practice levels through support for interprofessional team care
  • Supports strategic planning, develops presentations and provider-facing materials, engages stakeholders, presents to key audiences, and develops dashboards and graphics to report key metrics.
  • Creates and disseminates new workflows and approaches to care at regional and practice levels in support of interprofessional team care
  • Manages complex projects including issue resolution, project dependencies, and critical path
  • Ensures success of key initiatives as defined by the primary care leadership team
  • Utilizes data-based approaches to establish program goals and engages with clinical and operational leaders to ensure accountability and success in achieving program goals on schedule
  • Partners closely with subject matter experts, clinical and operational leaders, nursing leadership, population health, informatics and other key collaborators to promote systems redesign aligned with priority initiatives
