Career growth and development Diverse and collaborative working environment Affordable and comprehensive benefits package Position Summary Under the supervision of the Department of Social Work, provides direct supportive services and connections to recovery resources to adults in recovery from a substance use disorder. Serves as a role model, mentor, advocate, and motivator to individuals in order to promote long-term recovery. Works as a member of the care team by engaging patients in recovery planning, including identifying treatment services and facilitating referrals. Maintains contact by phone and/or email as well as in-person contact with recovering person after they leave the hospital to ensure their ongoing success. Provides re-engagement support with other agencies. Principal Duties and Responsibilities Performs and documents delegated patient care activities Performs patient care activities as defined in the skill competency and scope of practice Safely and independently completes patient assignment Identifies patients who need consult based on electronic health record Requirements High School Diploma or GED. PRC-focused education and training preferred. Must have lived experience of recovery from substance use. Must have skills to establish supportive relationships with persons experiencing substance use and co-occurring mental health disorders.
