Description UNC Continuing Care is actively seeking a skilled and compassionate Clinical Nurse to join our team. Summary: RN position coordinating Home Health, Hospice, Community Palliative Care and Home Infusion services for UNC Medical Center, and Hillsborough Hospitals. Educates patients and families in skilled services, how to administer infusion medications, and what to expect for services in the home. Works with Case Managers, Providers, and other Referral Partners to assist with discharge plans and care coordination to the community. Is also considered the Subject Matter Expert for all services provided by Continuing Care Services. This is an on-site position located at UNC Medical Center. Hours 8:30 AM to 5:00 PM Monday - Friday. Responsibilities: 1. Communicates all appropriate patient information and coordinates staff availability directly with the home health or hospice intake department. Communicates home health or hospice staffing availability to referral sources and staff.
