The Clinical Transformation Coordinator will work with Population Health leadership and
management teams to support the success of the population health programs, including but not
limited to performance improvement, governance, education, technology initiatives, and data
analysis. The role will participate in multiple cross functional teams to drive success of strategic
initiatives that improve patient engagement and quality of care delivered by Jefferson Health. By
supporting execution of such initiatives, the role will participate in decisions that have significant
impact on the care of patients and performance of the ACO/CIN/ Population Health Services and
value-based programs. Problems are expected to be complex and require careful consideration of
the needs of multiple stakeholders, performance goals, and fixed budgets.

Essential Functions
bull; Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
bull; Coordinates and supports strategic projects for ACO/CIN/Population Health Services
Supports implementation and optimization of Population Health initiatives to improve outcomes.
bull; Tasks may include managing vendor relationships, evaluating software or other solutions,
project management, workflow development, ongoing support and education.
bull; Participate in cross functional teams to optimize use of electronic health record and population
health information technology tools in practices.
bull; Administrates committees that are important to ACO and/or value-based programs.
bull; Tasks may include preparation of agendas and other materials, taking and summarizing
minutes, communicating action items, and ensuring execution of action items.
bull; Collaborates to develop policies, procedures, and workflows that improve quality and efficiency
of care for ACO/value-based program patients.
bull; Organizes and analyzes data in order to evaluate program effectiveness and identify
opportunities for improvement.
bull; Troubleshoot problems and inefficiencies; works with key stakeholders to develop solutions.
bull; Resolves and/or escalates operational issues raised by medical directors, practice engagement
liaisons, or key stakeholders related to assigned projects.
bull; May manage direct report(s) to support success in role and their growth.

of the organization change.
bull; Technical proficiency with the complete Microsoft Office suite required. bull; Professional verbal
and written communication skills.

Minimum Education and Experience Requirements:

Education:
bull; Bachelor#39;s degree in Business, Economics, Public Health or related field required.

AND

Experience:
bull; Five years of health care management experience.
bull; Experience working closely with key stakeholders including change management, practice
transformation, and workflow design/optimization required.
bull; Experience with payer quality measure programs and ACO models such as HEDIS, STARS, and
Medicare Shared Savings Program preferred.
bull; Experience with program management preferred.
bull; Experience with electronic health record(s) and/or other health information technology
tools preferred.
bull; Management experience preferred.

Since this position will require the selected candidate to engage in regular contact with children in the form of care, guidance, supervision or training, you will be required to meet the requirements of the Child Protective Services Act, including the Pennsylvania State Police criminal background check, the Pennsylvania Department of Welfare child abuse criminal background clearance, and an FBI finger print based federal criminal records check. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
