Join us...make a difference. 003707 PRIMARY FUNCTION: Under the general direction of the Population Health Management team the Program Coordinator participates and delivers services in a variety of Population Health initiatives. This position will provide assistance to both patient and health care teams for identification, eligibility and enrollment in programs as well as the monitoring and collection of outcomes data related to the various programs administered. ESSENTIAL FUNCTIONS: bull; Accepts referrals and manages assigned caseload for the identification and connection to community resources. bull; Determines insurance eligibility and assists in enrollment and any issues post enrollment. bull; Provide enrollees with information about the health insurance exchange and the health plans that are available. bull; Assists the care team in determining whether a patient qualifies for subsidies or Medicaid, and assists with the enrollment process. bull; Provides outreach and education about health-related social needs (insurance navigation, food insecurity, transportation) bull; Assists with the coordination and tracking of the various Population Health Initiatives bull; Performs audits/ reconciliation to ensure program targets and goals are met; running monthly Ad Hoc reports upon request; and notifying Population Health Leadership of identified process problems and/or audit outcomes. bull; Performs other duties as assigned. The selected candidate will not be allowed to commence working until all three (3) are completed satisfactorily.
