The Registered Professional Nurse at Northside Hospital independently plans and provides professional nursing care to patients in accordance with physician orders and established guidelines, and in accordance with the scope of practice as defined by the Georgia Nurse Practice Act. Serves as a main point of contact for Radiology outpatients (IR/CT/US patients and caregivers). Assists patients across the continuum of care for Interventional Radiology services. Serves as an essential link between patients, caregivers, and the multidisciplinary team. Establishes and maintains positive working relationships with physicians, nurses, physician office staff, case management, inpatient and outpatient staff, radiology, anesthesia, etc. Facilitates effective and efficient management of care including: a. Assisting with scheduling urgent cases, add-on procedures b. Coordinating transfer of patients between facilities c. Pre-procedure assessments d. Post procedure follow up phone calls e. Arranging for medical clearances as needed f. Providing instructions, education and directions for patients, caregivers and providers g. Addressing any patient questions/concerns as appropriate h. Coordinating scheduling of necessary follow up, imaging, etc. i. Tracking the need for repeat procedures 4. Monitors patient outcomes and participates in quality improvement activities. 5. Contributes to and collaborates with health care team members to positively impact patient outcomes and patient experiences. Facilitates a clear line of communication between referring physicians and care providers to avoid delayed treatment planning Follows-up with referring physicians, providing updates on the patients and families referred to radiology. Assesses patient's health literacy and potential barriers to learning. Provides patient and caregiver education about diseases processes, treatments, and procedures. Promotes and provides education to staff members as requested. Assesses patient's needs, identifying and documenting patient and family barriers to care. Provides access and information on resources available within the hospital and the community. Documents accurately and in a timely manner all patient, caregiver and multidisciplinary team interactions in the patient's chart. Participates in data collection process and records and follows up on treatment recommendations.: a. Quality data tracking b. Satisfaction survey c. Procedure volumes d. Follow-up recommendation Track and report clinical outcomes and other performance data as required/requested to improve quality of care and clinical outcomes. 13 Maintain and review patient records, obtain relevant radiographic images and other relevant reports performed for review prior to clinic visit. Maintains positive relationships between interventional physician practices and hospital, provides records and reports for physicians.
