Position Highlights:
  • Position: Care Coordinator MSW

  • Full time/Part time: Full Time, 40 hours/week
  • Hours: 8am - 4:30pm, Rotating weekends
  • Required Travel: No

What you will do

  • Implements assessment and care based on the developmental stages of the patient. Involves family/guardian in the plan of care and conducts patient and family education, as appropriate, considering the developmental and high risk needs of the patient and the impact of social/behavior issues on the illness and recovery process. High risk needs include, but are not limited to traumas, uninsured/underinsured patients, teenagers, substance abuse, domestic violence, undocumented patients, homelessness, and patients who are at risk due to inadequate support system.
  • Provides direct social work service and grief counseling to patients and their families in relationship to emotional problems, social and practical environmental problems in order to improve coping with illness related issues.
  • Identifies strengths and resources available to the patient and family; documents recommended actions, interventions or referrals.
  • Collaborates with multidisciplinary teams to develop and implement discharge planning for patients upon admission according to patient needs and payor requirements to ensure patient has an appropriate transitional care plan. In conjunction with the C3 RN, initiates the development of discharge plans for complex patients to eliminate discharge barriers as needed.
  • Collaborates with members of the healthcare team to determine goals for length of stay management and discharge management activities. Participates in SNAP huddles, discharge planning rounds, etc., as needed. Promotes cost-effective quality care services.
  • Identifies high risk, vulnerable populations and intervenes to coordinate care for these patients in treatment. Serves as a resource to those identified populations after discharge to promote health and access to treatment. Collaborates with interdisciplinary team in interactions with community agencies. Facilitates community linkage and referral on behalf of patient and assists in arranging medical follow up in order to reduce admissions.

What you will need:

  • Master's Degree from an accredited school of Social Work
  • Licensure to practice social work in Illinois: LSW/LCSW or LSW/LCSW eligible. Must obtain minimum of LSW within 90 days of hire
  • Minimum of two years recent clinical experience in the social work filed in an acute care setting; mental health setting and/or medical social work experience preferred
  • Current CPR certification a plus
