Description

This position provides case management to clients in a residential setting; by providing court advocacy, accessing medical care, assisting in accessing recovery-community supports, and obtaining necessary resources for independent living. This position also supports clients' transition from residential treatment, to outpatient, and into housing. The Case Manager will be familiar with, and able to access resources on behalf of clients and will be a strong advocate for accessing appropriate care and their recovery. This position is skilled in maintaining professional boundaries and providing direct services through a trauma-informed lens. This position has access to mental health billing and service records.

Location: 16th and Burnside Recovery Center

Schedule: Monday-Friday 8:00am-4:30pm

Compensation: Starting at $23.36/hour

CERTIFICATION MINIMUM QUALIFICATIONS:

There are multiple certifications which may qualify you for this position. To be considered, a candidate must have one of the following three:

1. Registered with MHACBO as a Certified Recovery Mentor OR Certified Peer Support Specialist (or Peer Wellness Specialist) AND be registered as a Traditional Health Worker on the THW registry.

Or...

2. Registered with MHACBO AND Relevant Bachelor's Degree OR 3 years relevant work experience towards QMHA.

Or...

3. Active credential with MHACBO as CADC-R or CADC

ADDITIONAL MINIMUM QUALIFICATIONS:



  • High school diploma, GED or demonstrated ability to read and write at a sufficient level.
  • Must possess a current driver's license and qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy. Must pass an initial driver training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage if using personal vehicle for CCC business.
  • The OAR prohibits A&D program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
  • Must meet CCC privileging requirements as required by FTCA.
  • Have, or able to obtain, Adult CPR Certification AND Adult First Aid Certification prior to start date.
  • Must have basic knowledge of substance use disorders.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
  • Will be required to carry a cell phone for work use. Cell phone to be provided by Central City Concern.
  • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit. Must be approved by the Childcare Background Check Unit.
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects.
  • Must be able to climb stairs several times a day.
  • Willingness and ability to work flexible hours, including some weekend and evening hours.
  • Must adhere to agency's non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, lifestyles, and sexual orientations and treat everyone with respect and dignity.


ESSENTIAL DUTIES AND RESPONSIBILITIES:



  • Provide case management services for clients residing at 16XBURN for residential treatment services, medical, mental health and SUD services while residing in 16xBurnside and following completion of residential treatment.
  • Develop, implement, and maintain a case plan to provide all wraparound services as needed for clients.
  • Develop and maintain a positive working relationship with all parties involved with clients' cases including, but not limited to, DHS caseworkers, VOA outreach workers, defense attorneys, physicians and mental health providers probation officer and other community partners.
  • Prepare case notes that clearly document all services provided for clients and maintain charts and records in a legible and accessible form.
  • Assist client in obtaining Primary Care Physician (PCP), if necessary.
  • Assist clients in any needed medical follow-up and transport them to their medical appointments.
  • Attend regular clinical meetings and provide current information to the team regarding client's participation in program and other activities.
  • Transport clients to appointments, hearings, support groups, and other services as needed.
  • Attend all client's court hearings.
  • Assist clients in applying for eligible benefits, including Oregon Health Plan, food stamps, cash assistance, housing assistance, childcare assistance, job training programs, etc.
  • Work closely with primary counselors, client access specialists in case planning; facilitating referrals to after care services, including housing, vocational rehabilitation, outpatient SUD treatment, mental health services, etc.
  • Participate in compliance, quality assurance and quality improvement activities as directed.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Attend all mandatory CCC trainings in a timely manner.
  • Perform other duties as assigned.
  • OTHER DUTIES IF CADC-R or CADC-I:


    • Cover SUDS related groups in the event the regular facilitator is unavailable and document activity in EPIC.


  • OTHER DUTIES IF QMHA-R or QMHA-I:


    • Cover Peer led groups, Skills Training and Development groups, or provide some limited Psychoeducational service, in the event that the regular facilitator is unavailable, and document that activity in Epic.
