Assist patients in managing their medication, utilizing the latest technologies, and achieving their personal health goals. Your expertise will significantly enhance the lives of those with diabetes and those at risk.
Job Description: Assess the diabetes education needs of patients and their support persons. Develop and implement an individualized education plan with the patient and care team. Provide diabetes education to patients, support persons, providers and staff. Support the diabetes education program and provide leadership in care improvement for patients with diabetes.

Pay Range Pay Range: $41.98 to $58.08 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable).
