Job Summary

This Health Care Navigator (HCN) role is part of a dynamic Care team within the Office of Population Health (OPH) Clinical Programs Pediatric Complex Care Team. This position reports to the Pediatric Supervisor, Pediatric Complex Care, under the direction of the Manager, Complex Care.

The incumbent will be a hybrid position requiring reporting on-site to various UCSF Health campuses including the OPH Brisbane office and visits to patient homes. When working from home, should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.

The Health Care Navigator will assist with navigating all aspect of the healthcare system. This team is part of the Novel Interventions in Children's Healthcare (NICH) Interventionist program (Pediatric Complex Care) that provides intensive non-clinical home, community, and medical system-based support, guidance and assistance for youth with chronic and/or complex medical conditions (e.g., type 1 diabetes, sickle cell disease, failure to thrive, pre-/post-transplant, etc.). Typically, these children are living in families with limited resources (primarily on youth covered by Medicaid and/or CCS-California Children's Services) as they navigate through complex healthcare and social environments. Primary goals are to increase patient satisfaction, and to optimize care and outcomes. The HCN works on building effective rapport with children and families so can act as a communication liaison to understand the patient's non-clinical individual needs, desires, and concerns. The HCN guides the patient and family to a broad range of local community and medical services, amenities, and information to promote health management and quality of life and ensure satisfaction with the patient care experience.

The Health Care Navigator serves as a non-clinical resource and liaison for patients and their families to help them achieve the best possible health outcomes for the specific chronic condition that makes them eligible for the NICH program (Pediatric Complex Care). Assists with navigating all aspects of the healthcare system as well as other systems which impact health outcomes. Uses knowledge of the healthcare and other systems to resolve issues and identify ways to increase patient satisfaction and outcomes. Collaborates with department leadership to meet patient expectations, solidifies positive family-provider relationships, address barriers to care access, achieves quality outcomes and builds a patient focused culture.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $55,200 - $95,900 (Annual Rate).

Department Description

The Office of Population Health (OPH) leads UCSF's development of innovative care models to improve the value of care and improve health outcomes for identified populations and facilitates implementation of these care models across the health system. OPH is responsible for fulfilling UCSF Health's accountable care contracts and fostering care partnerships across a clinical network. This department is comprised of three main segments defined as (1) Clinical Programs, (2) Quality and Value Based Care, 3) Analytics, and 4) Clinically Integrated Network and ACO Management.

Required Qualifications


  • BA/BS degree and a minimum of 2+ years previous experience in a healthcare related field OR an equivalent combination of education and experience (e.g., EMT, health educator, community health worker, social worker, health plan, medical corps and clinical office. staff.)
  • Two (2) or more years previous experience in a healthcare related field
  • Strong skills to comprehend and assess patient's grievances to quickly locate appropriate resource for assistance. In-depth knowledge of the organization and how to get issues resolved.
  • Strong interpersonal and customer service skills. Ability to communicate and resolve issues effectively with a diverse population of patients, staff and physicians.
  • Excellent analytical and problem-solving skills. Ability to develop solutions and recommend changes and follow through with implementation.
  • Excellent written and verbal communication skills.
  • Has the skills to work independently in a fast-paced, demanding environment with minimal supervision.
  • Ability to manage and oversee multiple tasks simultaneously, including high daily call volume

Preferred Qualifications


  • Knowledge of medical terminology.
  • Prior experience developing workflow procedures and implementing systems to improve operations.
  • Proficiency with EPIC APeX applications.
  • Experience working with vulnerable youth and families experiencing high social risk and/or health inequities
  • Strong knowledge of Patient Rights & Responsibilities, Joint Commission standards, and Centers for Medicare / Medicaid regulations. Strong knowledge of data collection, compilation, and analytical techniques.
  • Proficiency with Windows-based software including Microsoft word, Excel, Outlook. Knowledge of computer systems and software used in functional area.

License/Certification


  • Valid CA driver's license.
  • Meet all Hospital Risk Management requirements, including no moving violations within one year.
  • Must furnish proof of vehicle insurance and have reliable transportation.
  • Must have a personal vehicle.
