  • Portneuf Medical Center is a nationally recognized healthcare leader with 205 beds, serving as the region's Level II Trauma and tertiary care center and home to the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue.

Responsibilities

  • Under the general direction of the Diabetes Program Coordinator is responsible for the administration of community and staff education programs related to diabetes.
  • Consults with physician and other health care personnel to determine nutritional needs and diet restrictions of clients.
  • Assesses client's need for Diabetes education and implements individualized training plan, follow-up and behavioral goals.
  • Provides medical nutrition therapy and education to individuals or groups in specialty areas: Diabetes, Weight Management, Cardiac, eating disorders, gastrointestinal conditions, etc., adjusting teaching techniques with regard to age specific needs of audience.
  • Follows the standards of the Academy of Nutrition and Dietetics Nutrition Care Process when providing medical nutrition therapy for non-diabetes diagnoses.
  • Assists Program Coordinator in development and implementation of the yearly plan for community diabetes education program and in developing and maintaining the annual budget.
  • Implements class curriculum to meet needs of organization and regulatory bodies such as the American Diabetes Association.
  • Evaluates education programs and implements changes to improve performance.
  • Documents patient education, plan of care, goals and follow up in the medical record. Communicates the plan of care with HCP using SBAR communication skills.
  • Assists in the documentation and collection of data for ADA recognition.

Qualifications

Job Requirements:

  • Registered Dietitian through the Commission on Dietetic Registration or be registry eligible
  • Current Dietitian licensure through the State of Idaho
  • Current CPR certification

Preferred Job Requirements:

  • Certification in Diabetes Education (CDE)
