Perform Diabetes self-management education (DSME) and nutrition assessment. Provide comprehensive analysis of the assessment results to clinic endocrinologists and referring providers. Support and assist in coordinating follow-up care for patients with diabetes and complex medical needs.

Duties & Responsibilities:

  • Performs the duties of a Dietitian Nutritionist; assesses nutritional needs, diet restrictions, and current health plans; develops and implements dietary-care plans; and provides nutritional counseling.
  • Advises patients and their families on nutritional principles, dietary plans, diet modifications, and food selection and preparation.
  • Counsel individuals and groups on basic rules of good nutrition, healthy eating habits, and nutrition monitoring to improve their quality of life.
  • Consult with physicians and health care personnel to determine the patient's or client's nutritional needs and diet restrictions.
  • Assist with the care of patients with pre-diabetes and diabetes in an outpatient and telehealth setting.
  • Evaluate diabetes self-management skills in patients. This includes evaluating competencies in basic survival skills, advanced self-management of meals, exercise, sick days, and hypoglycemic and/or hyperglycemia events.
  • Monitor outcomes of DSME completion and present data for divisional and institutional review. Participate in efforts to grow the clinic education program in volume and reach.
  • Coordinates and leads diabetes education classes through the NWC education program.
  • Review available medical reports and assessments and provide treatment, safety, healthy food choices, and preparation.
  • Utilize education guidelines and modules to promote the optimal management of patients' diabetes control and skills.
  • Complete patient training education related to diabetes technologists, including insulin pumps, pens, and continuous glucose monitoring systems.
  • Coordinate insurance approvals and device delivery through interaction with industry vendors.
  • Perform other related duties and participate in special projects as assigned.


  • Minimum Qualifications Required:

    1. Bachelor's Degree (120 hours) in public, mental, and/or medical health education, social work, psychology, student personnel/affairs, nursing, education or communications
    2. Two (2) years (24 months) of experience conducting professional presentations, seminars or closely related programs and/or providing health education
    Note: An applicant with a Master's degree in an area of health education qualifies with one year of professional communications and/or health education experience

    Note: Possession of a Certified Health Education Specialist and/or Master Certified Health Education Specialist certification meets the requirements of #1 and #2 above. (http://www.nchec.org/)

    Preferred Qualifications:
    • Possession of current Registered Dietitian licensure from the Illinois Department of Financial and Professional Regulations
    • Certified Diabetes Care and Education Specialist (CDCES)
