Position Summary:
We have an exciting opportunity to join our team as aMultisite Director, Home Healthcare Intake *Home Care Experience and Certified Home Health Agency Experience Highly Preferred!!*Master's Degree Preferred*

In this role, the successful candidate Responsible for maintaining standards of practice and performance, the delivery of patient care in accordance with established standards of care, directing and facilitating work performance of personnel within the department and maximizing patient outcomes. The Director of Intake/Weekend/On-call serves as an advocate and a role model in the performance of direct and indirect patient care and assumes leadership role and function in absence of Administrator.

Job Responsibilities:

  • Performs other duties as assigned
  • Consistently addresses patient care needs in a timely manner as evidenced by, utilizing incident reports, being proactive on patient conferences, giving referrals to case management, social work, and screen inquiries and referrals and processes community requests for Home Care service in accordance with Agency policies and procedures. Collaborates with Intake Coordinators and other professionals both within and outside the Home Care Department to identify patient needs; develops strategies to meet the needs and evaluate the effectiveness of intervention.
  • Participates andor sponsors multidisciplinary process improvement activities for program specific and hospital wide initiatives. Assures unit compliance with JCAHO, DOH, NYS DSS and other regulatory agencies, as well as agency policies and procedures. Confers regarding reasonable budget projections for staff and supplies and effectively manages fiscal responsibilities.
  • Establishes and implements workflows which permit efficient processing of referrals. Does home health coordination, also known as intake coordination, to manage and facilitate the transfer of patients to the care of the Home Health Agency. Assists patients and families in obtaining information regarding private pay or third party coverage. Applies reimbursement criteria and regulations appropriately; monitors staff's application of adherence to reimbursement policies and procedures.
  • Assesses, evaluates and documents patient's responses in collaboration with nursing and interdisciplinary team interventions. Utilizes nursing theory. Performs patient home visits as indicated. Collaborates in assessment of patient's clinical status with Team Members and attempts satisfactory resolution of patient clinical issues in a timely manner. Collaborates with Administrator in attempt to have satisfactory resolutions with team member issues in a timely manner.

Collaborates with Administrator in identification, investigation and resolution of actual and/or potential risk management, quality care, ethical issues, or Team Member issues.   • Provides direct/indirect patient care utilizing WUH patient care guidelines. Assess potential and actual changes in level of care required. Documents plan of care to provide concise direction to all care givers. Utilizes knowledge of nursing theory, research and practice in the application of clinical judgment. Identifies patient satisfaction/dissatisfaction.
  • Encourages a functional, safe and therapeutic environment for patients and staff through regular orientation and education of staff on safety issues. Ensures staff compliance with Infection Control techniques both in practice and attendance at annual Infection Control in-service. Ensures a well-functioning, clean environment for staff. Performs patient home visits as indicated. Demonstrates a balance between proper levels of confidentiality/security related to patient records and interests with family staff concerns/interests.

Minimum Qualifications:
To qualify you must have a Associate's Degree Accredited Nursing Program. Current NYS Registered Nurse License R Professional Certifications / Registration: Indicate special certifications that are required or preferred for the position 1 - 2 years Nursing Practice and Community Health Nursing. . 2 years Nursing in a supervisory capacity in a certified CHHA & LTHHCP. . Excellent communication (both oral and written), interpersonal, organization, leadership skills. Proficiency with computers.

Preferred Qualifications:
Master's Degree Accredited Nursing Program.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $125,209.55 - $208,682.59 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
