RN-Resource Nurse/Team Lead (REMOTE)

Schedule- M-F 6:45a-5:15p

Responsibilities

  • Leads communication and coordination of care between all members of the care team.
  • Assists all patients and care team members throughout the care model by acting as a patient advocate.
  • Connects with providers and patients regularly via a telehealth platform and receives regular updates on patient activity.
  • Assists in the tracking and prioritization of high-risk patients.
  • Ensures that all new CCs and CPs have the appropriate tools and templates to operate.
  • Supports VCU manager in coordination of clinical and operational workflows across markets and documents those steps.
  • Interacts with the patient and the multidisciplinary team to evaluate and document measurable healthcare goals.
  • Communicates discharge information to other clinical departments or members of the care team.
  • Builds and maintains collaborative professional working relationships with all care team members.
  • Assists as needed in recruiting, interviewing and training of new team members.

Qualifications

  • Associate of Science in Nursing.
  • Current, active RN licensure in good standing and willing to be licensed in multiple states at Contessa's expense.
  • Three to five years of chronic care coordination. Possess a general knowledge of clinical standards and outcome measurement.
  • Must have reliable transportation and proof of liability insurance
  • Ability to support after-hours schedules and on-call needs.
