Job Overview:

The Clinician will assist patients in transitioning from inpatient and/or emergency room to outpatient care, including provision of/linkage to concrete services to facilitate enhanced community tenure. Services may be offered in the office or in the community. The Clinician will also facilitate psychoeducational groups for our Co-Occurring and Partial Hospital program's patients.

Qualifications:

Required:

  • Bachelor's degree in Psychology, Social Work or related Human Services field
  • 1-2 years experience in providing case management and/or clinical experience in a mental health setting
  • Valid state driver's license in good standing

Preferred:

  • Bilingual in English and Spanish

Scheduling Requirements:

  • Shift working, 8:30-4:30pm each day
  • Monday-Friday
  • FT

Essential Functions:

  • Screens new patients on the inpatient medical units and provides discharge planning services in a fast-paced, tight-deadline environment
  • Calls residential/outpatient substance abuse programs and faxing documents ASAP to secure intake appointments for patients before they are discharged
  • Provides intensive case-management to link clients to concrete social services including financial, transportation, housing, legal and related services
  • Collaborates with other professionals at the hospital as well as community agencies to meet the needs of assigned patients
  • Provides transportation for patients to appointments for social services, addiction treatment, medical appointments and housing
  • Provides outreach to patients who missed scheduled appointments
  • Following up on these referrals is the primary responsibility of this position
  • Screens new patients and identifies concrete service needs
  • Assists patients with applications to those services for which they may qualify, including Charity Care, Medicaid and other social service programs
