Job Description

  • Aids oncology patients in achieving optimal quality of life by focusing on symptom management, assisting with care coordination, providing support and education, empowering patients to optimize time with clinicians and facilitating appropriate follow-up care.

  • Provide Palliative Care/Coordination of Palliative Care throughout the continuum.

  • Serves as a liaison between the patient and family, primary care physician, internal and external care providers, specialists, referrers, support network members, e.g, social workers and nutritionists, and the wider health care community.

  • Leads, facilitates and collaborates with the multi-disciplinary team members across the enterprise to integrate evidence-based practices into the Pulmonary Health program.

  • Maintains contemporary knowledge of the management of pulmonary health patients both inpatient and outpatient, and of current clinical practice /other guidelines and regulatory specifications to include Joint Commission and other regulatory bodies. (Examples: reads and accesses resources, shares new developments, attends workshops and conferences.)

  • Supervises and controls all program-specific data generation for pulmonary health programs including IT/iCare reporting, vendors, patient-reported outcomes, and data registries. Partners with clinical data analysts/staff. Reviews and analyzes data regarding clinical outcomes and regulatory compliance. Provides data and reports to staff, physician leadership, enterprise leaders, and department director. Works closely with clinical analysts and IT/iCare staff to develop reports and improve data collection. (Examples:, Eon Dashboard statistics, CMS/TJC required measures, , and other pertinent data.)

  • Concurrently monitors the program's performance for inpatient and outpatient patient care units across the enterprise providing education and interventions with the clinical staff and providers as necessary to improve delivery of care.

  • Provides feedback to the teams and develops strategies to overcome organizational barriers regarding knowledge, process and interdepartmental collaboration. Additionally, brings frontline staff feedback to the multidisciplinary team.

  • Demonstrates teamwork and utilizes communication/ influencing expertise to foster collaboration among clinicians and providers for the adoption of best practices. (i.e., supports organizational objectives regardless of personal feelings, contributes to the greater good of the organization.).

  • Complies with regulatory standards and laws and strives to continue to advance the program. (i.e., maintains knowledge of current, changing and proposed standards and implements them in department, reviews policies and procedures periodically and revises, if necessary)

  • In collaboration with the department director, plans and directs services to improve clinical outcomes (i.e., plans and manages workflow to meet customer needs, develops operational plans, develops policies and procedures, , coordinates activities with others and communicates plans and trains staff as directed)

Qualifications

  • Master of Science Degree in Nursing required or Licensed Physician's Assistant .

  • Three (3) years' recent experience as a NP or PA in Outpatient Oncology setting or Pulmonary setting required.

  • Minimum of one (1) year outpatient experience.

  • Furnishing and DEA number.

  • Strong facilitation and collaboration skills. Ability to work cooperatively/collaboratively as part of a team, developing productive working relationships with diverse groups of people.

  • Smoking cessation program management experience preferred.

  • Proven critical thinking and problem-solving skills with the ability to organize, analyze and present data

  • Excellent communication skills both oral and written

License/Certification/Registration Requirements

Current California Registered Nurse Practitioner license or Physician's Assistant license required Current BLS certification. Current ACLS certification

Ages of Patients Served

This position will serve Adolescent, Adult, and Older Adults.

Salary Range:

$90.41 - $135.62 USD Hourly
