DaVita Integrated Kidney Care is seeking a Vice President of Chronic Kidney Disease and Value Based Care Capabilities (CKD and VBC Capabilities).

DaVita Integrated Kidney Care (DaVita IKC) is one of the largest end-to-end kidney care providers; we manage $5.5B+ in annualized medical spend through value-based partnerships. We provide comprehensive care and complex chronic condition management to 80k+ patients nationwide, whether they are in the early stages of kidney disease, transitioning to dialysis, seeking a kidney transplant or receiving life-sustaining dialysis.

Essential Duties and Responsibilities:

  • Reporting to the Chief Transformation Officer, the Vice President will support DaVita IKC's executive leadership team in achieving strategic goals with overall accountability and responsibility for Patient Engagement, Physician engagement, and Total Cost of Care Reduction for CKD patients via advanced primary care
  • Collaborate with the Chief Medical Officer and Group Vice President of ESKD Models of Care to deliver clinical innovations and excellent patient care
  • Accountable for engaging nephrologists in the DaVita IKC model of care via our high performing physician network (includes over 2400 nephrologists)
  • Lead the continued innovation and development of our CKD models of care

Minimum Qualifications:

  • Minimum of 5-10 years of progressive leadership experience in healthcare value-based care providers and/or health plan risk operations and/or at-risk payer services organizations
  • Operational or model of care design experience at a primary care value base care company
  • Leader of total cost of care reduction for primary care patients via advanced primary care / sustained chronic condition management
  • Operational experience with integrating care resources in physician practice offices
  • Deep knowledge of Hierarchical Condition Category (HCC) and Risk Adjustment Factor (RAF) by CMS with experience leading the operations of such a program
  • Knowledge of End of Life (EOL), Social Determinants of Health (SDOH), readmissions, and high-risk programs
  • Experience with patient engagement in value-based care (VBC) programs
  • Experience with healthcare data interoperability and designing tech-enabled workflows for clinicians
  • Proven capability to be results-oriented and lead at scale

  • Proven experience recruiting, developing and leading high-performing teams in a complex matrixed organization structure
  • Proven ability to identify and define business problems and issues and develop strategic and financial frameworks
  • Powerful communication and presentation skills, both written and verbal, including the ability to influence and impact results at all levels internally and externally
  • Bachelor's degree required; Master's degree preferred

Salary/ Wage Range

$175,000.00-$285,000.00 / year

Compensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience and may fall outside of the range shown.
