Project Healthy Grandparents

Project Healthy Grandparents (PHG) is dedicated to improving the well-being of families where grandparents are raising their grandchildren in parent-absent households. Since its establishment in 1995, PHG has positively impacted over 3,000 individuals, providing essential services such as home visits, parenting education, legal referrals, early intervention, and transportation assistance.

PHG, operating within the Byrdine F. Lewis College of Nursing and Health Professions at Georgia State University, is actively seeking a Sponsored Funded Community Services Specialist III, working as a Community Health Navigator, to join our team. The Sponsored Funded Community Services Specialist III, working as a Community Health Navigator, plays a critical role in supporting grandfamilies, particularly those facing health challenges. This position helps grandparents access healthcare services, resources, and provides advocacy and health education.

WHAT WILL YOU DO:

The Community Health Navigator works both independently and collaboratively with a team to coordinate services, assist with medical appointments, and improve the overall health and well-being of the families they serve.

DUTIES AND RESPONSIBILITIES INCLUDE:

Relationship Building & Outreach:



  • Build trusting relationships with grandparents raising grandchildren, assess health needs, and provide health education.
  • Conduct home visits and outreach to promote healthy behaviors and lifestyle changes.


Healthcare Access & Coordination:



  • Identify and connect grandfamilies with necessary healthcare services and resources.
  • Facilitate medical appointments, arrange transportation, and, when needed, accompany families to appointments.
  • Work within an interdisciplinary team to coordinate services that address the health needs of both grandparents and grandchildren.


Support & Advocacy:



  • Coordinate and lead support groups and educational workshops focused on nutrition, chronic disease management, mental health, and parenting skills.
  • Advocate for families within healthcare and social service systems to ensure they receive appropriate care and resources.
  • Collaborate with healthcare providers, social workers, and community organizations to meet the holistic needs of the families.


Documentation & Professional Development:



  • Maintain accurate records of services provided, interactions, and outcomes.
  • Participate in team meetings, trainings, and ongoing professional development to enhance knowledge and skills in community health, advocacy, and program delivery.



SPONSOR FUNDED:

***This is a regular, full-time position which involves on-campus and online duties, with job continuation dependent upon grant funding.

Qualifications :

Minimum Requirements:



  • Bachelor Degree and three years of related experience; or a or combination of education and related experience.


Preferred Requirements:



  • Experience working with vulnerable populations, particularly older adults, children, and families facing health challenges.
  • Knowledge of local healthcare providers, community organizations, and available resources for grandfamilies and children with medical needs.
  • Familiarity with health insurance programs and benefits, including Medicaid and Medicare.
  • Ability to speak a second language, particularly Spanish, to better serve diverse communities.
  • Experience in facilitating support groups or educational workshops focused on health and wellness topics.
  • Strong problem-solving skills and the ability to navigate complex social and healthcare systems.
  • Certification or training in health education, community health, or social services.
  • Demonstrated ability to work effectively in a multidisciplinary team environment.
  • Strong advocacy skills with experience working within healthcare or social service systems to secure resources for families.
