Work location: Remote The Senior Manager of Hospital Outreach will lead a team of Dialysis Transition Managers. These individuals are responsible for conducting outreach to hospitals in an effort to improve the transition of care experience for renal patients transitioning from the hospital to outpatient dialysis. This leader will drive change through others by helping to coach teammates in order to improve the quality of outreach and enable learnings and feedback from these key hospital stakeholders. They will be required to work cross functionally to identify, prioritize, and communicate the opportunities that will have the greatest impact on patient and hospital experience. The Senior Manager will work with a team of highly motivated and entrepreneurial individuals and is expected to: (1) effectively lead work streams with dependencies on many different business units, (2) collaborate well throughout the organization, and (3) communicate recommendations to senior executives.
