Purpose of Job: The Director of Population-Based Clinical Strategies is responsible for the development, oversight, and optimization of Ridgeview's population health initiatives. This role focuses on chronic care management, patient care coordination, and care transitions across acute and post-acute care settings. The Director will lead the Navigator and Value-Based program teams to drive care management improvements and expand initiatives that enhance patient outcomes and organizational performance. Additionally, this role will leverage predictive modeling, data analytics and social determinants of health (SDOH) insights to proactively identify at-risk populations and drive strategic decision-making. Job Functions:
- Provide strategic leadership and operational oversight for population-based clinical strategies, ensuring alignment with organizational goals.
- Manage and develop the Navigator and Value-Based program teams, fostering collaboration and effectiveness in care management initiatives.
- Enhance care coordination and transitions across acute and post-acute settings to improve patient outcomes and reduce readmissions.
- Utilize predictive modeling systems and data analytics to identify high-risk patient populations, inform interventions, and optimize care management strategies.
- Integrate social determinants of health (SDOH) into care management programs, ensuring a holistic approach to patient care and addressing barriers such as access to transportation, housing, food security and social supports.
- Develop strategies to connect patients with community resources and services that support overall health and well-being.
- Monitor and support key payor performance indicators, ensuring compliance with value-based care metrics.
- Collaborate with payors to develop contracting strategies that align with population health goals and financial sustainability.
- Optimize operational processes to support success in a dynamic reimbursement environment, ensuring efficiency and effectiveness in care delivery.
- Foster interdisciplinary collaboration across the organization to integrate population health strategies into clinical operations.
- All other duties as assigned.
Minimum Education/Work Experience
- Bachelor's degree in healthcare administration, nursing, public health, or a related field (Master's degree preferred)
- Minimum of 5-7 + years of experience in healthcare operations, population health management, or care coordination.
- Strong leadership experience with a proven ability to manage teams and drive program success.
- Experience with predictive modeling, data analytics and population health management tools.
- Knowledge of value-based care models, payor contracting, and healthcare reimbursement structures.
- Expertise in addressing social determinants of health and integrating them into care management strategies.
- Excellent analytical, communication, and problem-solving skills.
- Ability to work collaboratively with internal and external stakeholders to achieve strategic objectives.
