Purpose of Job This position will provide MTM services in ambulatory clinics; and may rotate through various community clinic settings within the Ridgeview clinic system. Job Functions Performs medication therapy reviews, pharmacotherapy consults, and medication management for Ridgeview clinic patients utilizing collaborative practice agreements and standing orders. Helps patients get the best benefits from their medications by actively managing drug therapy and by identifying, preventing and resolving medication-related problems. Assesses appropriate drug information and literature resources to provide effective information to varied audiences including patients, other health care professions, and peers. Engages in organizational and department quality improvement activities to improve patient care, medication use process, and pharmacy operations. Documents and maintains accurate medical records and reports. Educates and trains residents, students, interns, colleagues and other health care professionals. Supervises residents and students in their job tasks. Performs other duties as assigned. Minimum Education/Work Experience Bachelor's degree in Pharmacy Knowledge/Skills/Abilities Ability to work autonomously and as a team Ability to work carefully, methodically and accurately Ability to respond appropriately to unpredictable situations and diverse patient populations Capable of making independent decisions with the use of good critical thinking and problem solving skills Ability to meet and maintain the necessary background checks as aligned with position functions Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above License/Certification Valid Minnesota Pharmacist licensure Preferred Qualifications MTM Pharmacist experience MTM Residency training PharmD Link to Employee Health Physical Demands Compensation Many factors are taken into consideration when determining compensation such as: the requirements of the position, experience, education, knowledge, and skills along with location and internal equity.
