Description
The Care Manager reports to the Quality Division and is responsible for coordinating referrals and linkages to clinically indicated services. The Care Manager's role is helping members navigate and access community services, other resources, adopt healthy behaviors, improve health outcomes, and reduce healthcare costs. This role supports providers and Managed Care Coordinators through an integrated approach to care management and community outreach. The Care Manager supports the Managed Care Organization (MCO) and implements program improvements that strengthen care management, member access, and health outcomes.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Facilitates member outreach, engagement, linkage, referrals, coordination, and follow up for mental health, physical health, and substance use services and supports.
- Screens and assesses members for current mental health, physical health, and substance use disorders.
- Identifies gaps in care for members and arranges appropriate and timely resolution with internal and external agencies.
- Helps members engage with appropriate outpatient services to reduce member's visits to the Emergency Department, hospital admissions, and readmissions.
- Supports the mental and physical health care of members on an assigned caseload. Closely coordinates care with member's mental health, physical health, and/or substance use providers.
- Works closely with community resources and organizations to coordinate member services.
- Provides support and attempts to remove common obstacles and barriers to outpatient care.
- Identifies risk factors related to relapse and develops relapse prevention plans and strategies.
- Advocates for members who may be vulnerable or need additional support.
- Provides member education on common mental health, physical health, and substance use disorders.
- Provides brief interventions using evidence based psychosocial treatments, (CBT, MI) as clinically indicated.
- Participates in regularly scheduled caseload consultation. These consultations will primarily focus on members who are new to treatment, difficult to engage, or who are not improving as expected.
- Systematically tracks member follow up and clinical outcomes for reporting to the MCO.
- Supports medication management as prescribed by health provider(s), focusing on treatment adherence monitoring, medication education, and effectiveness of treatment.
- Collaborates with Managed Care Coordinators with the completion of Health Risk Assessments.
- Completes all documentation within assigned time frames.
- Other duties as assigned.
MARGINAL DUTIES & RESPONSIBILITIES
- May be required to work flexible schedules and be available to work on weekends or evenings as required. May be required to be on call.
EDUCATION
- Must meet the requirements for a Mental Health Professional (MHP) through one of the following educational paths:
- HS Diploma/GED + 5 years of supervised clinical experience
- Bachelor's degree in social work, psychology or closely related field
- Bachelor's degree in unrelated field + 2 years supervised clinical experience
- Master's degree in social work, counseling, psychology or closely related field preferred
EXPERIENCE
- Experience working with persons diagnosed with mental illness preferred.
- Ability to document services provided in an Electronic Health Record in a timely manner.
- Daily access to a personal vehicle is required.
- Must obtain and maintain $100,000/$300,000/$100,000 bodily injury and liability insurance.
- Subject to validation every six months.
- May be required to provide transportation for members of the program using a personal vehicle.
- Must be 23 years of age and must meet all insurance carrier's requirements.
- Subject to annual Department of Motor Vehicles license verification.
SKILLS/CERTIFICATIONS
- Current and valid driver's license.
- LCSW or LCPC preferred.
