The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Nurse Practitioner (NP)/Physician Assistant (PA), is responsible for providing primary health care to individuals and families who will be accessing care at Stout Street Health clinic, Stout Street Recuperative Care Center (SSRCC), and our other clinical sites This position is for a NP/PA for 40 hours per week, that will include 1 weekend day as part of their work week. The NP/PA would provide high-quality acute, chronic, and preventive care across the full age span, and works collaboratively with the SSHC multidisciplinary integrated health team.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
    • Provides direct, high-quality, primary medical care to people experiencing homelessness.
    • Breathes the spirit of Motivational Interviewing, Harm Reduction, and Trauma-Informed Care with clients, and provides information and choices in a non-judgmental, respectful manner.
    • Takes initiative to determine ways to deliver health care in a more compassionate, expedited, user-friendly manner.
    • Maintains a work environment that is marked by respect for others, that values inclusiveness and builds workforce diversity, and that fosters cooperation and teamwork.
    • Collaborates and consults effectively and bi-directionally with all CCH staff and identifies and addresses the complex issues which may adversely impact the health of patients.
    • Provides clinical precepting opportunities for providers-in-training as scheduling allows.
    • Maintains Continuing Medical Education, license (s), and CCH Credentialing and Privileging requirements always.
    • Delivers care across CCH sites which may include outpatient clinic sites, the mobile clinic, Stout Street Recuperative Care, street outreach or home visits.
    • Weekend /evening/Holiday coverage is required.
    • Performs other duties as assigned.
Qualifications Summary
    • 2 or more years of experience required for this job position.
    • Requirements for Nurse Practitioners:
    • A Master of Science or Doctorate degree in Nursing from a program accredited by the American Nurse Association or the American Association of Nurse Practitioners.
    • Current registration (or ability to obtain) in the State of Colorado as an Advanced Practice Nurse.
    • Certification as a Nurse Practitioner from the American Nurse's Credentialing Center.
    • Requirements for Physician Assistant
    • Graduation from a program accredited by the American Academy of Physician Assistants.
    • Current licensure (or ability to obtain) with the State of Colorado to practice as a Physician Assistant.
    • Current certification with the National Commission of Certification of Physician Assistants.
$109,220 - $131,064 a year

Licensed health care providers may earn funds towards student loans in exchange for a two-year commitment at an NHSC-approved site through this program.
[Language for RN/LPN postings only - Remove if not needed] Qualified candidates are eligible for the NURSE Corps Loan Repayment Program which offers payment of 60% of unpaid nursing education debt in exchange for a two-year commitment at a NURSE Corps approved site with an option to extend to a third year for an additional 25% of the original balance.
