Responsibilities

We are looking for a Social Worker to join our team within the After School Intensive Outpatient Program (IOP). If you are passionate about making a meaningful difference in the lives of adolescents facing psychiatric challenges and thrive in a collaborative environment, we invite you to apply for this rewarding opportunity! As a Social Worker, you will collaborate closely with psychiatric medical providers and therapists on the patient's primary treatment team to ensure a cohesive and effective plan of care.

Hours for this role are Monday - Thursday, 10a-730p and Friday 8a-430p

Responsibilities and Accountabilities:

  • Assessment and Planning: Conduct thorough assessments, maintain the care plan, and lead groups to enhance patient understanding and engagement.
  • Patient and Family Engagement: Maintain open lines of communication with patients and their support systems to involve them in treatment plan decisions and foster a collaborative approach to care.
  • Ethical Advocacy: Deliver and advocate for patient-centered, culturally appropriate, and age-appropriate care, ensuring that all services align with ethical standards and best practices.

Your expertise will be instrumental in guiding patients on their journey to recovery.


Qualifications

  • Master's Degree in Social Work with licensure in the State of Minnesota required.
  • Experience in healthcare setting preferred
  • Experience using electronic health records preferred
  • Proficient in Microsoft Office Products including Word and Outlook
  • Able to effectively and efficiently communicate in written and verbal form
  • Ability to smoothly and rapidly shift between dissimilar tasks
  • Ability to communicate well both verbally and in writing
  • Ability to react quickly and calmly as part of a team to handle volatile and potentially emergency situations
  • Ability to make effective decisions while in stressful situations and circumstances

Compensation for this role ranges from $26-$34 per hour depending on education and experience.
