POSITION SUMMARY:
To ensure staff productivity and efficiency. To develop and promote care coordination for social service referrals between the Valley Health System entities to improve patient outcomes. To provide program development for the VHC volunteer, bereavement and Journeys programs. Assists in on-boarding, mentoring and training of the VHC social service department new hires in coordination with the VHC education department. To provide education to VHC staff and community partners about mental and social determinants of health awareness. To screen, assess, and evaluate patients and their families to identify psychosocial and discharge planning needs and develop appropriate interventions. To educate, counsel, and refer as indicated. To plan and communicate effectively with multi-disciplinary team and community resources.
EDUCATION:
Masters degree and NJ License in social work from an accredited program required. New Jersey Social Work License or eligibility required. LCSW required.
EXPERIENCE:
5+ years of Previous medical social work supervision experience preferred. LCSW required
Occasional Field visits
Previous supervisor experience preferred

SPECIAL SKILLS:
New Jersey Social Work License or eligibility required. LCSW required. Effective group process skills. Effective oral and written communication skills to express ideas clearly. Effective listening skills. Ability to follow-through using appropriate channels of communication. Ability to pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and the changing work environment related to changing patient and staffing needs. Ability to work cooperatively with patients, family members, and multidisciplinary team members (i.e. nurses, physicians, Rehab, Chaplains, HHAs, Art Therapist, etc.). Ability to utilize effective time management to set priorities, perform job related responsibilities, and respond quickly to emergency situations. Ability to use effective analytical and critical thinking skill to problem solve.
