Job Overview:

The Clinician provides daily case management, support and advocacy to consumers with mental health and/or substance abuse needs in a culturally sensitive and patient-centered manner.

Qualifications:

Required:

  • Bachelor's degree in Social Service field
  • Valid NJ Driver's license
  • Commitment to working with clients with compassion and empathy for underserved populations
  • Knowledge and understanding of urban issues, hospital procedures, community resources
  • Strong verbal and written communication and organizational skills to effectively work and communicate with clients, service providers, hospital staff and TRAC team
  • Computer literacy, punctual and reliable
  • Able to work in stressful situations and problem solve, including verbal deescalation of consumers

Preferred:

  • 1 year of case management experience
  • Clinical experience working with the behavioral health population, crisis intervention, and/or case management in a hospital setting
  • Bilingual in Spanish or Haitian Creole

Scheduling Requirements:

  • Shift working, 8:30-4:30pm each day
  • Monday-Friday
  • FT

Essential Functions:

  • Attends staff meetings, workshops, trainings and supervision
  • Conducts various assessments and screenings, not limited to: biopsychosocial, service plan, suicide risk, depression and anxiety
  • Consults with other service providers and resources to best meet consumer needs
  • Facilitates psychoeducational groups for consumers
  • Maintains compliance with all organizational policies and procedures
  • Maintains consumers records by reviewing case notes; documenting contacts and progress
  • Manages and coordinates service (treatment) plan, making amendments as needed
  • Other responsibilities as assigned by management
  • Provides on-call services for crises and emergencies
  • Provides pre-crisis and crisis support in person and over the telephone
  • Provides transportation to clients for BH and medical appointments, social services and other needs as required
  • Supports, encourages and collaborates with the consumer and his/her family, providing care coordination for behavioral health, physical health needs, and social needs
