Job Summary
Clinical Licensed Social Worker or Social Worker Based on qualifications, selected candidate may be hired at either Clinical Licensed Social Worker or Social Worker Job Summary UCDH Home Care Services Licensed Clinical Social Worker: services are provided to individuals and caregivers in their place of residence for the purpose of promoting, maintaining or restoring health, or minimizing the effects of illness and disability. The Licensed Clinical Social Worker (LCSW) provides comprehensive clinical social services that are appropriate to the needs of the individual patient, family, and caregivers, and functions as an integral member of the interdisciplinary team. UCDH Home Care Services Social Worker: services are provided to individuals and caregivers in their place of residence for the purpose of promoting, maintaining, or restoring health, or minimizing the effects of illness and disability. The Social Worker 2 (SW2) provides comprehensive clinical social services that are appropriate to the needs of the individual patient, family, and caregivers, and functions as an integral member of the interdisciplinary team. - Clinical Licensed Social Worker
- Required to hold a valid California driver's license, have a driving record that is in accordance with local policies/procedures, and/or enroll in the California Employer Pull Notice Program.
- California Licensed Clinical Social Worker (LCSW).
- Valid California Board of Behavioral Sciences Licensed Clinical Social Worker license without restrictions.
- American Heart Association (AHA) Basic Life Support (BLS) or UC Davis Health CPR
- Master's Degree (MSW) in social work from an accredited school of social work.
- Experience within an acute medical hospital, mental health setting, and/or health care clinic.
- Basic computer skills.
- Ability to read, write, speak, and understand the English language. Excellent written and verbal communications skills.
- Ability to utilize medical terminology appropriately.
- Ability to remain calm under pressure.

- Demonstrates accountability and professionalism.

- Ability to make appropriate interventions based on law and professional ethics.
- Ability to provide consultation, interpret clinical findings and diagnosis, make recommendations regarding treatment, care, and disposition.
- Ability to use clinical supervision to enhance professional growth and examine clinical work.
- Ability to work independently with minimal supervision. Strong critical thinking and problem-solving skills to manage multiple levels of information and responsibilities, and to quickly assess complex problems to develop potential solutions.

- Knowledge of suicidal behavior, the ability to assess lethality, and the ability to develop appropriate safety plans.

Preferred Qualifications

- Specialty social work certification from accredited organization such as the NASW, ACMA, etc.
- Two (2) years post-graduation experience.
- Prior home health, palliative care, and/or hospice social work experience.
- Proficiency with computer programs and experience with electronic medical records (EMR).

Department Overview Home Care Services (HCS) is made up of three primary programs: Hospice (inpatient and home care), Home Health, and Home-Based Palliative Care (HBPC). The human services team also includes a Bereavement Program, a Home Environment Assessment Program (HEAP), as well as a Medical Aid in Dying Program (MAID). This position would support all programs. Work Environment UC Davis is a smoke and tobacco free campus effective January 1, 2014.
