Breast Health Navigator

The Breast Health Navigator is a nursing practice role that serves patients with breast cancer and their families throughout the HealthPartners integrated health system. The Breast Health Navigator identifies the needs of patients with breast cancer from diagnosis throughout the care continuum, providing consultation and collaboration with the interdisciplinary team of providers (oncology, surgery, radiation therapy, and radiology) to assist these patients and their families in achieving their health care goals. Breast Health Navigator care includes assessment, intervention, and ongoing evaluation of the physical and psychosocial symptoms, care needs, and outcomes of patients with breast cancer; provision of patient education and counseling; design and delivery (written and presentation) of educational materials; identifying and assisting patients in obtaining resources within the healthcare system and community. This position will also support for other areas of the Breast Center as needed. Performs other duties as assigned

Work Schedule: Casual 0 FTE; Per diem, day hours

Required Qualifications:



  • Associate degree Nursing
  • Minimum of 1 year experience in the care of oncology patients or clinic experience.
  • Expertise in physical and psychosocial assessment.
  • Licensed as an RN by Minnesota Board of Nursing
  • Wisconsin RN License (If not present at hire, must obtain within 3 months)
  • Current BLS - American Heart Association or BLS - American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
  • Able to prioritize, delegate and be self-directed.
  • Strong clinical, human relations, leadership, and verbal and written communication skills.
  • Ability to effectively solve problems and make sound decisions.
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
  • Proficient in the operation of the following: personal computer utilizing word processing, graphics package, slide projector, overhead projector, photocopier, telephone, and fax machine.
  • Proficient in pager, voicemail, and email use.
  • This position requires familiarity and compliance with matters of law, regulations and internal policies affecting the employment relationship, (including but not limited FMLA, ADA, discrimination, and harassment laws).


Preferred Qualifications:



  • BSN
  • Oncology Certified Nurse (OCN) certification or CBCN (Certified Breast Care Nurse) or CN-BN (Certified Navigator - Breast Care Nurse)
  • EPIC
