Linkage to Care Specialist - HIV Treatment

Purpose

Provide intensive outreach and linkage to care case management services to person's living with HIV who are newly diagnosed, at risk of falling out of medical care, or recently released from incarceration. Services provided will, including assessment, service plan development, coordination of medication care referrals to internal and community supports and may take place in a clinic, hospital, and community-based clinic settings.

Primary Functions

  • Responsible for the management of referred newly diagnosed, out of care, at-risk, and recently incarcerated people living with HIV.
  • Provides intensive linkage to care case management services patients referred to the Linkage to Care Program. Complete intake and assessment. Develop and execute appropriate service plans, care coordination, prevention and health literacy education.
  • Assumes responsibility for coordinating linkage to outpatient care based on patient insurance status and medical need working in compliance with State of Wisconsin Linkage to Care practice standards.
  • Works in partnership with Froedtert Emergency Department and Urgent Care Clinics to provide notification of positive HIV test results for patients tested in these settings.
  • Collaborates with community partners, including County health departments providing public health support with the goal of reducing new HIV diagnoses in Milwaukee and surrounding counties.
  • Provides intensive education around HIV disease prevention, progression and treatment.
  • Maintains detailed and confidential medication records in accordance with established protocols.
  • Demonstrates proficiency in the use of the electronic medical record (EMR).
  • Ability to effectively and efficiently negotiate healthcare and social services in addition to providing care coordination in the community to facilitate linkages for newly diagnosed HIV and high-risk patients.
  • Serves as a team member on multidisciplinary medical care team, communicating with team members to ensure comprehensive and quality wrap-around support.
  • Collaborates with inpatient staff, community partners, medical case management staff and Partner Services to support retention of care.




Preferred Schedule:




Full-time role with expectations for coverage during core business hours and flexibility required as necessary to accommodate business needs.



Position Requirements:

Knowledge - Skills - Abilities

Knowledge of HIV disease prevention, treatment and disease progression. Familiarity with HIV intake and case management practices. Comfort in working in both a clinic and community setting. Strong interpersonal and de-escalation skills. Familiarity with HIV treatment practices. Experience working in a clinical setting with a multidisciplinary team. Excellent oral and written communication skills. Fluency in health insurance systems and navigating health care systems. Experience documenting in Electronic Medical Record systems. Strong critical thinking, problem solving, and attentiveness to detail required. Ability to manage several high acuity events at one time.

Appropriate experience may be substituted on equivalent basis

Minimum Requirements:

  • Bachelor's Degree
  • 1-3 years experience in community support setting

Preferred Requirements:

  • Master's degree in public health, Social Work, or other related field
  • 3 or more years of experience working with HIV patient populations; Bilingual in Spanish and English

Certification:

  • Within first year of hire: Motivational Interviewing, Counseling Testing and Referral, and Screening Brief Intervention, Referral and Treatment (SBIRT) required within the first year of hire.
