Enrollment Support Specialist - Community Care Partners

Job Summary:

As a key member of the CCP Management Team, and specifically the Intake Department, the Intake Support Specialist assists with the overall goal of promoting integrated care coordination and improving health outcomes for enrollees of the Massachusetts' Community Partner (CP) Program. The department supports the Care Teams of CCP's Member Organizations in the provision of care coordination for MassHealth members with complex medical, behavioral health and long term supports and services (LTSS) needs who are enrolled in an Accountable Care Organization (ACO). This role partners with the Director of Operations and other Intake Coordinator(s) to support CP enrollment and disenrollment functions across eight Member Organizations and interfaces regularly with our ACO partners. The role specializes in data management as related to enrollment, transfers and disenrollment.

This is a non-exempt position.

Job Duties and Responsibilities:




  • Manage/triage a high volume of incoming correspondence and data from a variety of internal and external stakeholders.



  • Manage and maintain information and data to properly store, and share information with internal and external stakeholders.



  • Create and/or maintain informational databases to enter data, produce reports and analyze outcomes regarding referrals, enrollment and care coordination documentation.



  • Receive new referrals to our BH and LTSS CP programs, assist referral sources to complete referral process and determine eligibility for CP program.



  • Track enrollment status of all enrollees and reconcile with MassHealth CP enrollment, MassHealth eligibility system (EVs) and other platforms and update care teams.



  • Receive care coordination documentation, upload to electronic health record and update care team.



  • Partner with Points of Contact of internal care teams and external ACO/MCO partners to improve the procedures to manage referrals, enrollment, disenrollment and care coordination documentation exchanges with Community Care Partners.



  • Provide training to CCP and direct care staff regarding all areas of Intake including referral procedures, new enrollee procedures and care plan tracking.



  • Provide affirmative customer service when managing questions and concerns with timeliness and professionalism.



  • Maintain knowledge of the Community Partner program and effectively offer information and clarification.



  • Perform other related duties as required.





Requirements:




  • Bachelor's Degree required, preferably in Human Services, Healthcare Administration, or related field.



  • Minimum of two years of experience in data management and/or customer service.



  • Fully remote role, hours of the role somewhat flexible.



  • Excellent organizational and time management skills including data management.



  • Demonstrated proficiency in MS Office including advanced skills in Microsoft Excel, Google Sheets or similar program.



  • Strong interpersonal and customer relations skills.



  • Professional oral and written communication skills.



  • Ability to exercise judgment & discretion in dealing with individuals and confidential matters.



  • Ability to handle multiple responsibilities in a fast paced and challenging environment.



  • Ability to work independently and as part of a team.



  • Experience with databases such as Access, or SQL programming is a plus.
