The Manager Patient Navigation supports the Oncology Patient Navigator program system-wide, by way of collaborating with multidisciplinary teams and tumor site specific committees to ensure high quality of care across all of the hospital's cancer treatment sites. This program helps patients get the health care and other resources they need to be as healthy as possible by working with patients, their families, and their caregivers to overcome barriers to cancer screening and diagnosis, cancer care, and resources needed after cancer treatment. Working under general supervision, this position serves as a single point of contact and resource for all oncology sites and providers to coordinate services and provide consistent and effective care to patients. |

Establishes and maintains a positive working relationship with key customers (physicians, nurses, office managers, ancillary services, and key stake holders) to ensure effective and efficient management of care across the continuum of care. Provides a continual communication plan to physicians, ancillary services, office managers, and key stakeholders. Assists in the development of standards and methodology to identify patients at the appropriate point in their care journey to receive coordinated care and services provided by the Patient Navigator. Develops standards for follow up for patients and transition points to survivorship programs or end of line. Provides support for the Navigation Program in meeting organizational goals and objectives to promote departmental philosophy, develop annual program goals and monitors program/quality outcomes. Monitors/reports compliance with departmental policies and procedures and coordinates departmental orientation, continuing education, and clinical development to enhance program/clinical outcomes. Performs other duties as assigned. |
EDUCATION: |
Required: Bachelor of Science in Nursing Preferred: Master in Nursing |
EXPERIENCE: |
Required: 3 Years as a Registered Nurse (RN) and proven leadership in a clinical setting |
LICENSURE/CERTIFICATION/REGISTRY/LISTING: |
Required: RN | Registered Nurse licensed in North Carolina or a Compact state; National certification appropriate to area of specialty |
