Clinical Social Worker lll Psych-ZSFG-Adult-CWCM Full Time 85310BR Job Summary The Clinical Social Worker III (CSW III) is a licensed clinician providing psychosocial assessment, crisis intervention and 5150 services, individual and group therapy and substance abuse services, brief therapeutic interventions, consultation, education, community linkages, supportive services, and facilitates the plan of care for patients with particular focus on high-risk populations. This position involves much outreach to the community including client homes, hospitals, shelters, and jails as well as to the streets. The CSW III acts as a consultant to the clinical team, service lines, community partners, participates in program development and quality improvement initiatives. The incumbent will demonstrate cultural sensitivity in working with patients regardless of race, gender identity, religious affiliation, cultural beliefs, sexual orientation, etc. The incumbent will practice within the scope of UCSF's mission

Under the general supervision of the Clinical Supervisor and/or Program Director, the CSW III works within the innovative Division of Citywide Case Management Programs, which utilizes a proactive, client-centered approach to engage historically disengaged clients in the community. This program is responsive to the needs of the Mental Health San Francisco (MHSF) initiative and is fluid in service delivery based on the needs of the Behavioral Health Services division under the Department of Public Health. This program provides intensive case management to adults who have severe mental illness and/ or substance use disorder and are struggling with homelessness, histories of incarceration and other psychosocial issues. This program will often focus on clients that are struggling to engage in other systems of care and require the increased outreach and intensive support. The CSW III position will support with assessments and triage of client needs from Office of Coordinated Care, ZSFG/Psychiatric Emergency Services, Behavioral Health Court and the Criminal Justice system and from other referral sources. The CSW III will work collaboratively with the Adult/Older adult System of Care within Behavioral Health Services to develop creative treatment strategies. Through personalized CARE plans and dedicated support, the program endeavors to empower individuals to manage their mental health challenges effectively and to facilitate their integration into suitable pathways for sustained treatment and support.
