**Available to regular status employees who are scheduled to work a minimum of 24 hours. Southcoast Health is an Equal Opportunity Employer. Responsibilities Under the direction of the Executive Director, Pharmacy or designee, establish and maintain liaison relationships with the medical and office staff in a SPN primary care practice and other settings. Ensure the safe and effective use of medications by assessing for optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services. Provide Medication Reconciliation and Medication Therapy Management tasks as needed, documenting on appropriate media and seeking reimbursement where possible. Monitor medication therapies and work with practice staff as appropriate to identify alternatives, ensuring provision of best possible evidence-based care. Provide education to staff and support to pharmacy program management and assist in the development of best practices and maintenance of cost-effective pharmacy programs. Provide one-on-one consultations both in an office setting and at patient homes if indicated. Qualifications
Compensation: Pay rate will be determined based on level of experience. Pay Range USD $120,556.80 - USD $151,278.40 /Yr. |
