POSITION SUMMARY:

The Medical Case Manager is responsible for assessing the needs of assigned patients caseload and identifying barriers to care, including comprehensive insurance, risk assessment, medical, adherence, and treatment needs; developing service plans based on those identified needs; facilitating referrals and access to social service benefits and other internal and external resources and other providers. Monitors patient progress and advocates on behalf of patients when they are unable to do so for themselves; documents in the electronic medical record and complies with all data entry, data integrity, and data tracking requirements for BMC and funding agencies contract monitoring reports.

JOB REQUIREMENTS

EDUCATION:

Master's Degree in social work, psychology, or related human services field (equivalent experience may substitute for degree).

LCSW, LICSW or LMHC as appropriate, preferred.

EXPERIENCE:

Experience working with HIV/AIDS clients, and related issues, including co-morbidities.

Effective verbal and written communication skills in English. Bilingual-bicultural is a plus.

KNOWLEDGE AND SKILLS:


  • Expertise in establishing working relationships with patients, their families, professionals, and the community.
  • Knowledge of community, social, and health resources with particular knowledge of oncology-related resources.
  • Facility performing multicultural assessments and interventions.
  • Sensitivity to ethnic, cultural, gender, and sexual orientation diversity, values, beliefs, and behaviors.
  • Demonstrated ability to be flexible with client's priorities, evolving needs, and goals.
  • Demonstrated empathic understanding of underserved patients who are diagnosed with cancer, and offer have other co-morbidities.
  • Demonstrated ability to write contract reports that are clear and informative.
  • Demonstrated critical thinking and problem solving ability.
