Experience Treating Co-Occurring Disorders Preferred

(Master's degree, i.e., MA, MC, MSW, in a Behavioral Health Field)

Arizona Licensed: LCSW, LPC, LISAC, LMSW, LAC, LASAC or LMFT in Good Standing

Under the Agency's policies and professional requirements, with oversight by the Program Supervisor, the Justice Services Assessment Clinician works closely with the Maricopa County Adult Probation Department, Bridging the Gap (BTG) Re-entry and Crisis Aftercare Programs, the Terros Health Service Team, and other collateral community contacts to engage justice and crisis referred clients. The engagement process includes conducting a comprehensive biopsychosocial assessment to determine the diagnosis and individual treatment needs of the client. The Assessment Clinician may complete assessments for a variety of referral sources, including MCAPD, the Department of Corrections, local and county jails, the BTG Re-entry and Crisis Aftercare Programs. The Assessment Clinician participates in the development of the Service Plan through the service team format, which strives to effectively meet the patient's/family's needs and aims to result in improve health outcomes. This position is responsible for providing evidence-based, therapeutic interventions which may include outpatient assessment, diagnosis, and counseling services for adults. The position reports to the Supervisor of BTG Re-entry or Director of Justice Services

Essential Duties:



  • Prior to the assessment, reviews the all referral and supplemental documents to gain insight about the patient being assessed. This may include Pre-Sentencing Reports, Public Access Case Lookup, Probation Officer insights, etc.
  • Conducts a comprehensive biopsychosocial and ASAM Criteria assessment to identify whether the patient requires treatment, and if so, the treatment level of care and supportive services necessary to meet the patient's needs.
  • Requests additional information from the referral source if the determination from the assessment is that treatment is not recommended; including discussions surrounding the original reason the referral was made and confirm that no additional information is available that would influence the determination, as evidenced by documentation in the patient record.
  • Coordinates with the referral source after each assessment. This includes, but is not limited to, communicating treatment recommendations, any concerns that arose in the assessment, and to make referral to an appropriate facility, should the ASAM Criteria assessment indicates a need for stabilization services (medical detoxification), inpatient, and residential services.
  • Upon completion of the assessment, prepares a written assessment summary that details findings of the assessment and appropriate treatment recommendations based on these findings.
  • Completes coordination of care activities as needed with other entities to ensure individual client needs are identified, documented in the assessment, and addressed in the Individualized Service Plan.
  • After each assessment, the Assessment Clinician shall coordinate care for the patient and connect them to appropriate Terros Health team members based on their referral source and identified needs.
  • If designated, conducts therapeutic services using evidence-based interventions. Interventions may include individual, couple, family or group service methods.

Qualifications

  • Must possess a Master's degree, i.e., MA, MC, MSW, in a behavioral health field, Arizona behavioral health license (other than LBSW) or eligible for Arizona license from Arizona Board of Behavioral Health Examiners.
  • Must have ability to perform first aid or cardiopulmonary resuscitation as necessary and appropriate.
  • Knowledge in use of Electronic Health Record. NextGen a plus.
  • Proficiency using the MS Office suite, particularly Word and Outlook.
  • Must maintain a valid Arizona driver's license, be 22 years of age with minimum 3 years driving experience, and meet requirements of Terros Health's driving policy.
  • Must pass background check in accordance with Terros Health and DCS requirements which includes, but is not limited to, no probation or parole within the last 10 years at time of assuming role.
  • Must have Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass DCS Central Registry check without any disqualifying events.
  • Must pass a TB Test.
  • Experience working with diverse populations.
