Position: Complete Health Nurse (RN or LPN)

RN Salary Wage Range: $87,000-$90,000 annually, depending on experience

LPN Hourly Wage Range: $26.00 - 32.00 per hour, depending on experience

Service Area: Northwest Metro (coverage area: Cokato, Winsted, Excelsior, Buffalo, Champlin, Loretto, Anoka, Osseo, Rogers, Elk River, Rogers, Ramsey, Maple Grove, Zimmerman, Albertville, Bethel & surrounding). Plus, mileage reimbursement of .70 cents per mile.

Schedule: 1.0 FTE, Monday - Friday, 8:00am-5:00pm (no holiday, on-call or weekend rotation)

The Complete Health Nurse is a key member of the Lifespark COMPLETE team that serves Seniors in their homes. The Complete Health Nurse serves as a nursing expert who plays a critical role in onboarding new members to the program, supports medical needs throughout their time on service, and supports members during vulnerable periods of great need. Complete Health Nurses support the delivery of expert geriatric primary care by taking a whole-person approach to delivering home-based medical services. They will work collaboratively with a broader interdisciplinary team of providers and other allied health professionals to improve their members' health.

Position Functions & Responsibilities: **some duties/responsibilities may not apply to the LPN role



  • Provides medical oversight, including medication management, wound care, and education on managing chronic conditions like diabetes or heart disease.
  • Monitors vital signs and identifies early signs of health decline to prevent hospitalizations.
  • Conducts health assessments (including annual Wellness Visits and Health Risk Assessments)
  • Evaluates fall risk factors related to health conditions or medications and educates members on managing symptoms
  • Educates caregivers on medication administration, wound care, and symptom monitoring.
  • Completes interventions following an acute care episode to reduce risk of readmission or ongoing inpatient utilization
  • Performs procedures such as basic wound care, procedures such as Nasopharyngeal testing, ear lavage, medication/vaccine injections, phlebotomy, injections, glucometer use, catheterization etc.
  • Conducts in depth medication reconciliation
  • Provides care coordination and facilitates referrals as needed
  • Proactive outreach to members to engage them in appropriate services
  • Development and revisions of a tailored service delivery plan that addresses members' needs, challenges, and desired outcomes


Additional Functions:



  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse/licensed practical nurse. Completes required oversight documentation and escalates assessment needs to medical team as needed
  • Completes orientation and annual training requirements assigned
  • Participates in Lifespark's Event Management process including initiating reports, escalation of events and investigations
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training
  • Schedules and keeps member appointments
  • Organizes travel efficiently and submits accurate auto use logs by established deadlines
  • Tracks all billable activities



Qualifications:

Education



  • Registered Nurse or Licensed Practical Nurse
  • Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology


Experience



  • 2 years' RN/LPN experience in community-based services (assisted living, home care, hospice, public health)
  • Experience working with seniors preferred
  • Experience in customer service/direct member contact field preferred
  • Experience in multiple settings and situations
  • Experience in long-term goal setting and proactive intervention
