Overview

Nurse Practitioner-TEAM LEAD

Responsible for providing mentorship and subject matter expertise to the team of clinicians and for performing direct clinical tasks. This role utilizes an in-depth knowledge of the clinical team's operations to provide advice and insight on workflow questions or challenges.


Responsibilities

  • Facilitates team collaboration and the alignment of goals in a mentoring capacity.
  • Validates and coordinates the work of other clinicians on the team.
  • Collaborates with the Clinical Program Director to influence decisions related to their area of expertise and to contribute to the development of processes and procedures.
  • Assists the Clinical Director with onboarding, training, and ongoing coaching for clinicians.
  • Assists the Clinical Director and Recruiter in recruiting and hiring additional team members.
  • Reinforces organizational policies and procedures, evidence-based best practices, and regulatory/industry changes.
  • Works with patients and caregivers to assess goals of care, document advance directives, and make appropriate treatment plans based on goals.
  • Monitors care plans in conjunction with the rest of the clinical care team.
  • Evaluates medical needs for a specific caseload of patients and provides clinical care.
  • Advocates for the patient and caregivers in liaison with other care providers to ensure care is both coordinated and consistent with the patient's goals.
  • Participates in the measurement and assessment of care-related processes and outcomes to improve the quality of patient care.
  • Communicates plan of care to primary physicians and other consulting clinicians.
  • Educates patient/family on trajectory of illness, use of medications (including reviewing side effects, how to administer, and purpose), and assesses disease understanding.
  • Participates in the weekly IDT meeting, together with the rest of the care team.
  • Participates in the market's provider on-call schedule.
  • Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
  • Performs other duties as assigned.

Qualifications

Required:

  • Master's degree in Nursing.
  • Current, unencumbered license to practice as a nurse practitioner specific to the state that the employee is assigned to work by the company. Must have the authority to prescribe as a Nurse Practitioner in the state where services are provided. Must meet the training, education, and experience requirements to be authorized to practice as a Nurse Practitioner in the state where services are provided including, without limitation, being certified as a Nurse Practitioner by a recognized national certifying body that has established standards for the profession. Must meet Physician-Nurse Practitioner restrictive collaborative requirements specific to state assigned to work by the company and will supply a copy of all active collaborating physician agreements and indicate whether these will be transferrable to your employment with Amedisys.
  • Four (4+) years of experience as a nurse practitioner.
  • Advanced Certified Hospice and Palliative Nurse (ACHPN) certification; if not currently certified, must be willing to obtain certification within six (6) months of hire.
  • Current CPR certification.
  • Reliable transportation and company required liability insurance.
  • Valid driver's license.

Preferred:

  • Two (2+) years of experience in palliative care and/or geriatrics.
  • Experience in mentorship or coaching or prior leadership experience.
  • Experience in value-based care and/or managed care.
