Summary:
Responsible for monitoring the medication use process for the organization. Collaborates with all health care professionals, staff and leadership and considers all components of the medication-use process in all settings to improve medication safety.

Essential Functions:
-Identify and prioritize opportunity areas for improving medication safety for all practice settings involved in the medication use process.
-Implement standardized processes for improving medication safety.
-Develop criteria for and documentation of actions/changes taken/implemented to improve medication safety. This includes plans for follow up audits with documentation to ensure changes implemented have the desired positive outcome, improve medication safety, and that the implemented changes are being sustained.
-Engage staff and managers in maintaining and continually improving medication safety.

Education:
Doctor of Pharmacy Pharmacy/Pharmaceutical Studies required

Licenses and Credentials:
MA Pharmacist

Experience:
- Experience as a Clinical Pharmacy or in a Medication Safety role 2-3 years (required)

- Experience with data evaluation and regulatory oversight 2-3 years (preferred)

- Pharmacy leadership experience 2-3 years preferred (preferred)

Knowledge, Skills and Abilities:
- Demonstrated leadership with superior communication (both verbal and written).
- Strong presentation skills utilizing various types of media.
- Ability to use independent judgment.
- Analytical skills necessary to identify and resolve problems relating to drug therapy.
- Strong analytical skills and an understanding of statistics, population data, and the concepts of risk and prioritization.
