A Brief Overview

This position is an entry level Community Health Worker responsible for providing introductory support to Care Transitions staff members, University Hospitals Patients, Providers and Community Health Worker Partnered Sites.

What You Will Do



  • Develop individual, trusted, relationships with patients via telephonic and electronic means focused on building rapport and general assessing of social barriers to care:
  • Coordinates with patient to assess and identify appropriate community resources needed. Coordinates schedules to ensure timely and accurate patient visits and to prevent missed visits.
  • Actively interacts with, Initiates and maintains ongoing communication with Social Worker, RN, care team and Provider to facilitate patient care coordination. Serves as a liaison between patient and patient's family when appropriate and patients internal/external care team.
  • Proactively addresses anticipated patient barriers to care
  • Addresses missed, declined, unverified, and rescheduled visits with patient to assess for barriers to attendance of appointments
  • Understands PHI and HIPAA and actively protects patient confidentiality and uses appropriate points of contacts and consenting
  • Communicates with Patients as they matriculate throughout the system.
  • Demonstrates ability to collaborate among peers and system resources. Advocates for patients needs professionally and is capable of representing the patient as an advocate.
  • Is able to identify patient safety concerns and collaborate with Social Worker to address high level concerns.
  • Refers only complex and/or clinical issues to Social Worker for follow up
  • Demonstrates knowledge of patient history and provides high level over of patient history based on chart research.
  • Manages daily, weekly assigned tasks, documentation, follow up calls and reports.
  • Maintains professionalism .and community health worker standard of professionalism and ethics in high stress situations and complex cases.
  • Demonstrates interpersonal and communication skills, and emotional intelligence when working with patients/caregivers and clinicians at all times.
  • Consistently uses technology resources to track patient progress on developed plan.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
