LPN - School-based Asthma Therapy

Overview: 0.8 FTE, 32 hours/week year-round. Day shift; negotiable start time between 7am and 8am. LPN will travel to assigned patient schools during some part of workday. Valid Ohio driver's license and proof of auto insurance is required by hospital policy. Must pass a motor vehicle background inspection, insurance eligibility, driving qualifications and training set forth by Nationwide Children's Hospital. LPNs with the School-based asthma therapy (SBAT) Program perform asthma follow-up visits at different schools, communicates related needs for child's medication regimen, identify needs for home and school medications reconciliation, maintain records, initiate changes, transport medications, and coordinate asthma care between home, school, and patient's asthma care provider.
