This RN Administrative Coordinator is primarily responsible for reviewing all incoming patient referrals for home health, nursing and therapy services, daily scheduling of home health float staff, and patient care coordination during while a patient is pending their home health start of care or resumption of care.
RN Administrative Coordinators are responsible for the programmatic management of Continuum Home Health and Home Infusion. This includes the implementation of the home health program, quality review, quality improvement, intake and referral review and orientation. Nurse Coordinators work closely together as members of the leadership team, and with an interdisciplinary group of clinicians, including RNs, PTs, OTs, SLPs, MSWs, Home Health Aides, and other departments within the agency and UVA Health. RN Coordinators are a local resource for clinical field staff and patients, answering questions related to clinical practice and patient care.
MINIMUM REQUIREMENTS Education: Graduate of accredited nursing program, Bachelor of Science in Nursing required within 5 years of hire. Experience: 3 years relevant Registered Nurse experience required. Previous home health, home care, or home infusion experience highly recommended. Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. American Heart Association (AHA) Health Care Provider BLS certification required. Active driver's license required. PHYSICAL DEMANDS Job requirements: None required. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs.
