SUMMARY
Make a meaningful impact in a role that blends compassionate care with critical safety. As a Patient Safety Monitor - CNA at Grady's Crestview Health & Rehabilitation Center, you'll support patients during a crucial stage of their recovery journey - helping them regain stability, safety, and independence.
Under the supervision of a Registered Nurse, Licensed Practical Nurse, or Physician, you'll provide continuous monitoring for patients on suicide precautions, those under involuntary mental health status (1013, 2013, 1014, 1021), and others at risk of falls or self-harm. You'll ensure a safe, secure environment while responding to patient needs with empathy, vigilance, and professionalism.
In addition to safety monitoring, you'll deliver hands-on care as a Certified Nursing Assistant-supporting activities of daily living, providing nourishment and comfort, and assisting with delegated therapies. This dual-impact role is ideal for CNAs who are passionate about patient advocacy, mental health awareness, and making a difference through safety and care.

Job Responsibilities
Prioritizes seamless care by ensuring patient handoff to designated nursing personnel before any breaks or end-of-shift transitions. Remains with patients during transport or movement, as directed by licensed staff, to maintain safety at all times.
Recognizes and responds to behavioral changes with confidence-identifying signs of agitation or aggression, using the least restrictive interventions, and applying physical restraints using proper technique when necessary.
Acts quickly and communicates clearly-reporting changes in patient condition and interventions to licensed staff promptly and accurately. Maintains independence and reliability while functioning with minimal supervision.
Delivers focused and uninterrupted observation, ensuring safety is never compromised. Stays fully engaged-avoiding distractions such as TV, radio, or personal devices-while maintaining supervision and compliance with patient safety protocols.
Provides compassionate, hands-on support, including direct patient observation, personal care, and assistance with daily needs to promote comfort, dignity, and well-being.
Champions patient rights by advocating for patients and families, addressing concerns, and elevating issues to supervisors. Contributes to a culture of excellence by participating in performance improvement initiatives, task forces, or committees as needed.
Qualifications
To be considered for this role, candidates must meet the following requirements:
High School Diploma or equivalent - Required
Current American Heart Association Basic Life Support (BLS) - Healthcare Provider certification - Required
Certified Nursing Assistant (CNA) certification in the State of Georgia - Required
