GENERAL SUMMARY:

The Social Work Specialist supports and maintains the psychosocial health of members through assessment and resource coordination. The Social Work Specialist provides basic casework and consultation for members and facilitates communication between members, their family, and applicable healthcare and community resources.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Assesses and counsels applicable members regarding Advance Directives and their options to document their wishes and encourages members to prepare and complete Advance Directives and document within their medical record.
  • Assist and counsels members enrolled in a Case Management program with health related financial, social, and emotional concerns.
  • Provides consultation to the Case Managers regarding the impact of psychosocial factors on illness.
  • Evaluates client's medical and social needs and develops plan of care/goals appropriate to findings.
  • Monitors patient/family/caregiver ongoing functional/psychosocial status. Acts as an advocate for the member and family throughout the entire episode of care.
  • Instructs members/families/caregivers on community resources, facilitates referrals as appropriate and maintains availability to the member/family as a resource for communication with community agencies and to monitor services rendered.
  • Collaborates with the member's interdisciplinary care team, which may include their primary care physician, specialists, home care providers, and other designated individuals to effect timely, appropriate care coordination to ensure desired outcomes are achieved.
  • Enters data into clinical/psychosocial assessment tools designed to compile member data and assist with measuring outcomes.
  • Ensures and enters documentation of social work case management activities in a timely manner and in accordance with professional standards to ensure member continuity of care and compliance with any regulatory requirements, using appropriate computer applications.
  • Effectively communicates with internal and external customers, establishing and maintaining positive relationships.
  • Proactively initiates interventions designed to optimize patient outcomes /goals and address barriers.
  • Utilizes problem-solving techniques and conflict resolution skills. Identifies and manages problem cases. Effectively communicates with individuals, departments, and other entities for timely resolution of the identified problems.
  • Maintains strict confidentially standards for the member's information. Complies with organizational, federal, and state regulations and policy on confidentiality.
  • Performs other related departmental duties as assigned.

EDUCATION/EXPERIENCE REQUIRED:

  • Master's degree in Social Work.
  • Unrestricted State of Michigan license.
  • Competent in the provision of medical social work, including evaluation and assessment.
  • At least three (3) years of social work experience in a health care setting, preferable in a case management setting.

    CERTIFICATIONS/LICENSURES REQUIRED:

  • State of Michigan licensure in Social Work required.

    Abilities Required:

  • Proficient in computer skills and Microsoft applications.

  • Must work effectively with persons of varying position levels and diverse interests to reach consensus on resolutions of problems.

  • Must be able to effectively engage members/their family and other caregivers.

  • Possess excellent written and verbal communication skills.

  • Possess excellent problem solving skills, decision making and organizational skills.

  • Ability to work autonomously and as a member of a team.

  • Knowledge and experience with community based resources.
