Health Outreach Specialist (Hybrid - Troy, MI) - Health Alliance Plan

GENERAL SUMMARY:

Under the direction of the Manager of Government Programs, the Health Outreach Specialist will provide telephonic and face-to-face support to members identified for the Community Health Worker (CHW) program, assist with the development, coordination, implementation and evaluation of quality improvement and health promotion programs, and coordinate services and provide linkages to community based resources to address members' social determinate of health needs.

PRINCIPLE DUTIES AND RESPONSIBILITIES:

  • Assist with the implementation of outreach strategies for members to facilitate compliance with established programs and guidelines.
  • Track performance and outcomes associated with outreach efforts.
  • Coordinate and distribute the maternal infant health program referrals and reports monthly to the appropriate agencies.
  • Educate members regarding the health outreach and care management programs available at HAP Empowered.
  • Educate and inform members on preventive health topics, coverage updates, and community-based resources and services.
  • Manage and follow up on the Healthy Michigan Plan Health Risk Assessments daily with providers and members.
  • Collaborate with other organizations, agencies, vendors and/or community groups in developing outreach initiatives as appropriate.
  • Ensure that all initiatives increase member health as well as comply with standards for the National Committee for Quality Assurance (NCQA) and the Michigan Department of Health and Human Services (MDHHS).
  • Provide interventions for targeted populations through outbound telephone calls and face-to-face visits to members or clinics related to care gaps and barriers to care.
  • Respond to incoming inquiries for the Care Management team via telephone system, email, faxes and/or incoming mail.
  • Assist member with making health care appointments, such as PCP, specialist, and preventive health tests.
  • Provide and document individualized member education as directed by manager and within scope of practice.
  • Assist member with navigation of HAP insurance system.
  • Act as community liaison between HAP Empowered and community organizations.
  • Maintain knowledge of community resources to assist members in accessing support services and state and federal government programs.
  • Perform other related duties as assigned.

EDUCATION/EXPERIENCE REQUIRED:

  • Associate's degree in health related field preferred.
  • Minimum of one (1) years' experience in health care or managed care organization.
  • Community Health Worker certificate or a willingness to earn a certificate preferred.
  • Experience coordinating multiple tasks preferred.
  • Experience working with Medicare and Medicaid programs and/or experience in related community services preferred.
  • Proficiency with windows-based software, including Outlook and Excel preferred.
  • Experience or familiarity with community support services preferred.
  • Excellent verbal and written communication skills preferred.
  • Proficient organizational skills preferred.
  • Demonstrated ability to work independently preferred.
  • Excellent interpersonal skills preferred.
