The Advanced Practice Provider (APP) & Manager for the Mobile Hospice Response Team, MHRT, serves a dual role, providing clinical care within the inpatient palliative medicine setting at JHH, while also managing the Mobile Hospice Response Program in partnership with Gilchrist. The position blends direct patient care with strategic program oversight, requiring both clinical expertise and leadership capabilities.

In the clinical capacity, the APP works collaboratively within a multidisciplinary team to deliver comprehensive, age-appropriate care to seriously ill patients and their families. Responsibilities include consultative assessments, symptom management, therapeutic guidance, and support for prognostic awareness. The APP ensures that medical care plans align with the patient's goals and addresses both physical and psychosocial needs. Documentation of consultations, follow-up evaluations, and prescriptive recommendations are maintained in the electronic medical record.

As manager of the MHRT, the APP oversees all administrative and operational activities of the program. This includes the clinical oversight of the MHRT nurse liaisons assigned to MHRT within JHH. The APP collaborates closely with the Gilchrist Medical Director and the JHH Director of Clinical Programs, Palliative Medicine to develop and implement clinical operations, policies and procedures. They also coordinate educational initiatives for internal and external staff and serve as the primary care provider for selected hospice patients within JHH.

This integrated role requires a high level of clinical acumen, leadership, and the ability to navigate complex systems to ensure compassionate, coordinated, and patient-centered care.

Specific Duties & Responsibilities:



  • Deliver comprehensive care to patients with serious illness, including assessments, diagnostics, and treatment planning in collaboration with referring physicians.
  • Manage acute and chronic symptoms, while coordinating medication needs and therapeutic adjustments.
  • Provide patient and family counseling on disease progression, care options, and complex medical decision making.
  • Document clinical encounters in the EMR and facilitate communication with referring providers and interdisciplinary teams.
  • Demonstrate working knowledge of healthcare regulatory standards, including The Joint Commission (TJC), CMS, and hospice specific guidelines such as Part A Medicare coverage.
  • Operate independently while managing multiple priorities across clinical and strategic domains.
  • Exhibit strong leadership, interpersonal, and influencing skills to collaborate with physicians, payers, regulatory agencies, and interdisciplinary teams.
  • Plan, implement, and monitor achievement of strategic business objectives for the program.
  • Communicate strategic initiatives to key stakeholders while contributing to marketing efforts, program growth, and relationship management to support long term sustainability and expansion.
  • Maintain a flexible work schedule (5-8 hour days or 4-10 hour days) based on program and individual needs.



Minimum Qualifications:

Advance education either: Master's degree in nursing or physician assistant training; Must be licensed as NP in State of Maryland. Minimum of three years of related clinical APP experience preferred.



Licensure and Certifications:

Successful completion of the national certification examinations. Current CPR certification required. Maryland CDS and DEA licensure required. Familiarity with regulations and standards governing APPs in the State of Maryland. Must maintain current licensure and certification during duration of employment. Completion of the JHH credentialing process is required prior to start date.

Salary Range: Minimum /hour - Maximum /hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
