Access to Care is under the Center for Health Improvement, a corporate MaineHealth department, that supports programs and services aimed at making our patients and communities healthier.

The role is an in-person, salaried position that support admitted in-patients at Maine Medical Center at 22 Bramhall St. in Portland, Maine. The team has shifts between the hours of 7am and 530pm Monday through Friday and has a rotating schedule to provide coverage on Saturdays. Remote work is not an option. The team meets with patients and their families at the bedside to screen for program eligibility for various community, state and federal programs and then assists with all steps of the application process. These programs include Medicaid (MaineCareand all subcategories), Marketplace, SNAP, PHIP, long term care, disability, Medicare, Free Care (MaineHealth Charity care), etc. The team plays a key part in the discharge planning process by submitting long term care applications for patients that will need to discharge to a facility.

Summary:
The Lead Case Manager - Medical Outreach role is responsible for acting as the senior Case Manager on the team to be used as a resource and guide to all other team members. In addition to the functions of a Medical Outreach Case Manager carrying a caseload, the Lead is the main point of contact for management when dealing with matters of budgeting, staffing, strategizing, and problem solving.

Required Minimum Knowledge, Skills, and Abilities (KSAs)


  1. Education: Bachelor's Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred.
  2. License/Certifications: Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver's license and reliable vehicle required.
  3. Experience: With a Bachelor's Degree: two years of experience in a community health setting, including one year of case management experience required. In lieu of Bachelor's Degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
