The ideal candidate will possess the following qualifications:

• A valid Driver's license and reliable vehicle: This position requires travel to various schools, homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
• Must be able to work 9am -4:30 pm Monday-Friday

Educational and Experience Requirements

• Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
• Registered nurse; or
• A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years' experience in direct contact with mental health consumers

Skills preferred

• Experience in case management or related human services field preferred.
• Strong organizational and communication skills.
• Ability to work independently and manage a flexible schedule.
• Commitment to person-centered care and community-based service delivery.

We are seeking a Community-Based, Adult & Child Behavioral Health Case Manager to join our Program in Warminster, PA, serving Clients in Lower Bucks County.

The Community-Based Behavioral Health Case Manager is responsible for providing comprehensive case management services to individuals and families in the community. This includes assessing client needs, developing care plans, connecting clients with resources, and advocating on their behalf. The goal is to support client stability, independence, and improved quality of life through individualized, culturally sensitive, and strengths-based services.

The Case Manager will:

• Provide case management services in the location where the individual resides or requires support.
• Develop individualized service plans within 30 days of admission; review and update plans every 6 months or as goals evolve.
• Maintain face-to-face contact:
  
  
  
  • Every 14 days for individuals requiring intensive case management.
  • Every 30 days for individuals receiving resource coordination.
  
  
  
  
• Meet productivity requirements as directed by management within the 37.5-hour work week.
• Participate in a rotating weekly on-call schedule (compensated).
• Attend all required annual trainings per regulatory and management guidelines.
• Participate in scheduled meetings and supervision sessions regularly.
• Maintain accurate documentation in compliance with MERAKEY, county, and state regulations (including charts and vouchers via paper or Avatar system).
• Monitor EVS for assigned caseload to ensure proper billing.
• Adjust work schedules, approaches, and caseloads to meet the evolving needs of individuals served.
• Perform other duties as assigned that support the mission of the organization.

Work Environment:

• This position requires travel to client homes, shelters, and community agencies.
• May require occasional evening hours depending on client needs.
• Office and field-based responsibilities; this is not a work from home position

Earn $21.75 per hour, or $23.75 per hour with the selection of our Enhanced Pay option!
