Primary Responsibilities:
- Conduct in-depth assessments of patient needs, goals, and barriers to achieving good health outcomes. - Provide advanced health education and coaching to individuals and families to promote healthy behaviors and self-management of chronic conditions. - Collaborate with healthcare providers to develop and implement care plans for patients with complex health needs. - Assist patients in navigating the healthcare system and accessing appropriate services, including health insurance and social services. - Provide ongoing follow-up and support to patients to ensure continuity of care and successful achievement of health goals. - Develop and implement health promotion programs and activities to address community health needs. - Collect data and maintain accurate records of patient interactions and outcomes. - Attend meetings and trainings related to community health promotion and education. What You'll Bring Requirements: - Bachelor's Degree in Public Health, Social Work or related field of study. Experience in lieu of degree can be accepted. - 2+ years of experience in community health outreach, health education, or related field - Valid Driver's License Preferences: - Demonstrated ability to work effectively and provide advocacy for all populations and communities. Schedule and Work Model - Full time (40 hours) Monday through Friday, 8am-4:30pm - Hybrid - Field travel to meet with patients in the community as needed Mass General Brigham Community Physicians, Inc. is an Equal Opportunity Employer.
